BOARD OF COUNTY COMMISSIONERS
AGENDA REQUEST
DATE: December 2, 2014
TO: Honorable Chairman and Members of the Board
FROM: Elliot Kampert
SUBJECT: Request for plan amendment and rezoning, T. Davis Gordon
DEPARTMENT: Growth Management
BCC DISTRICT: 1
PUBLIC HEARING: Consideration of a request changing the use of land submitted by T. Davis Gordon
dba Tellus Development, Ltd., relating to property located on Livingston Street, Crestview. The request is
to change the Comprehensive Plan Future Land Use Map (FLUM) designation for the property from
Mixed Use (MU) to Commercial (C), or a more restrictive FLUM designation. If the FLUM amendment
is approved, request to rezone the property from Mixed Use (MU) district to General Commercial (C-3)
district, or a more restrictive zoning district. Property contains 1.31 acres, more or less.
STAFF FINDINGS:
• The applicant would like to expand his existing mini-warehouse storage business onto adjacent
parcels.
• Mini-warehouse storage is only allowed in the General Commercial (C-3) zoning district.
• The MU zoning district allows C-3 permitted uses when the property involved fronts upon an
arterial or collector roadway, and there is no access to the commercial part of the property
through a residential area or residential-zoned area.
• The property involved fronts upon Livingston Street which is not classified as an arterial or
collector roadway.
• The C-3 zoning district allows access from roadways not classified as arterial or collector.
• The applicant is requesting the FLUM change and rezoning so as to allow access from Livingston
Street to enable the expansion of his mini-warehouse storage business.
• There are other commercially-zoned properties in the immediate vicinity as well as properties
classified as “vacant commercial” by the Property Appraiser.
PUBLIC COMMENT/OPPOSITION: None to date.
PLANNING COMMISSION: The Planning Commission voted unanimously to recommend approval at
a public hearing held November 13, 2014.
MILITARY REPRESENTATIVE: No comment.
STAFF POSITION: Staff has no objection to the requested FLUM amendment and rezoning.
RECOMMENDATION: It is recommended that the Board consider the facts presented herein, the
Planning Commission recommendation, as well as any facts that may be presented at the public hearing,
and then decide whether or not to adopt the requested FLUM amendment and rezoning by separate
ordinances.

Guidance for Businesses with Employees
Returning from Countries Currently Experiencing
an Outbreak of Ebola Virus Disease
Contents
This document provides guidance for businesses with employees returning from countries
currently experiencing an outbreak of Ebola Virus Disease (EVD);
http://www.who.int/csr/disease/ebola/en/.
Introduction
• The Centers for Disease Control and Prevention (CDC) and the Department of
Homeland Security's Customs & Border Protection (CBP) have begun entry screening at
five U.S. airports that receive all travelers from the Ebola-affected nations of Guinea,
Liberia, and Sierra Leone.
Trained CBP staff will observe travelers for signs of illness, ask them a series of health and
exposure questions and provide health information for Ebola and reminders to monitor
themselves for symptoms. Trained medical staff will take their temperature with a non-
contact thermometer.
• If the travelers have symptoms or the health questionnaire reveals possible Ebola
exposure, they will be evaluated by a CDC quarantine station public health officer. The
public health officer will again take a temperature reading and make a public health
assessment. Travelers, who after this assessment, are determined to require further
evaluation or monitoring will be referred to the appropriate public health authority.
• Travelers from these countries who have neither symptoms/fever nor a known history of
exposure will receive health information for self-monitoring.
• The CDC will distribute contact information for these screened passengers to the state
they are residing in, or traveling to, via Epi-X.
• Upon receiving the notification, the Florida Department of Health (DOH), Bureau of
Epidemiology, will immediately distribute passenger lists to the county health
departments (CHDs) of residence for monitoring as described below.
• Please refer to the Interim Guidance for Monitoring and Movement of Persons with
Ebola Virus Disease Exposure for recommendations for conditional release and
controlled movement of persons potentially exposed to EVD.

Packet Pg. 33
What businesses should do:
Notification of local CHD for returning employees
• To ensure that DOH is aware of persons returning from the Ebola-affected countries by
all conveyance types, businesses should notify the local county health department that
an employee will be returning from the impacted country and supply contact information
including name, telephone numbers, address, and email address. In addition,
businesses should also provide the CHD with estimated arrival date and date employee
left impacted country.
• The CHD will make contact with the person(s) on the list and conduct an in-person risk
assessment on their date of arrival in Florida using the DOH EVD risk assessment
algorithm 3.5.
• An airline employee (pilot or crew) that during the course of travel never left the airport
grounds and never stayed overnight in an impacted country OR a employee on a ship
who never disembarked from the ship while in an impacted country AND have had no
contact with a known or suspected EVD case will not be monitored for 21 days.
However, employees should self-monitor and if they develop any symptoms report this
to their local CHD.
• Definition of employee risk categories
o Low risk: Employee returning from Guinea, Liberia, or Sierra Leone who did not
have contact with a known or suspect EVD case in the past 21 days.
o High risk: Any direct contact or within 3 feet for ≥ 15 minutes of a known or
suspect EVD case in the past 21 days regardless of the absence/ use and type
of personal protective equipment.
• The employee should be provided a 24/7 phone number to contact appropriate CHD
staff.
• Employees that are considered low or high risk will be monitored by the local CHD for 21
days as identified in the CHD Guidance-Traveler Monitoring document.
If you have any additional concerns or questions please contact your local CHD or the
Florida Department of Health at 850-245-4401.
Additional Considerations: In most cases, low risk employees are able to carry on with day to
day activities, although the individual circumstances of each person should be considered.
Factors that may prevent someone from returning to daily activities during the monitoring period
include their ability to quickly seek immediate medical attention if they develop symptoms,
number of daily close contacts, and risk of transmission.

FROM: Sunil Harman
SUBJECT: Final Consultant Contract Payment
DEPARTMENT: Airport
BCC DISTRICT: All
STATEMENT OF ISSUE: The Board of County Commissioners is requested to approve final
contract payment to Airport Management Resources, Inc. (AMRI)
BACKGROUND: The contract with AMRI for airport management consulting services expired
on September 30, 2014. This company has served the County Airports system faithfully and
exceptionally for nearly 17 years. During that period John Tomec has demonstrated
unconditional loyalty to our Airport Department with his unique ability to find innovative
financing opportunities. He was solely instrumental in getting the Airport into the FAA Military
Airports Program on four separate occasions resulting in over $20 million of financing for
projects that would not otherwise be eligible for regular Federal funds. John has decided to
begin his transition into retirement. His advice and wise counsel will be missed. This final
payment under Task Order No. 35 is for $6,000 through the final date of his contract.

SUBJECT: Clennan Family Foundation Hangar Lease Supplemental Agreement Two
DEPARTMENT: Airport
BCC DISTRICT: All
STATEMENT OF ISSUE: A proposed Supplemental Agreement Number Two to the Clennan
Family Foundation Hangar Lease, Block 4 Lot 2 at the Destin/Ft. Walton Beach Airport, is being
presented to the Board of County Commissioners for approval.
BACKGROUND: On August 9, 2012, the Board approved an Assignment of Lease for
Clennan Family Foundation, Mr. Roy Clennan, for a hangar located on Block 4 Lot 2, at the
Destin/Ft. Walton Beach Airport. Mr. Clennan has submitted his final invoice for the renovation
of his hangar totaling $68,237.50. Staff reviewed the submitted invoice and determined that the
cosmetic work, routine maintenance, and the personal workshop/office are not eligible for
consideration and would not be included in the total price of the renovation to determine the term
extension. Staff determined and approved an extension of the lease for (4) four years based on
eligible improvements. Mr. Clennan agreed that the new expiration date of the hangar lease will
be extended to May18, 2030. Mr. Clennan's certificates of insurance are attached along with the

SUBJECT: Direct Payment of October 2014 Airgas Invoice
DEPARTMENT: Public Safety
BCC DISTRICT: All
STATEMENT OF ISSUE: Request approval for direct payment of the October 2014 Airgas,
Inc., invoice in the amount of $3,576.73.
BACKGROUND: Airgas, Inc., is the sole medical oxygen vendor for Okaloosa County
Emergency Medical Services. Historically, invoices for oxygen services have been paid by
after-the-fact purchase order. “After-the-fact” meaning the department did not submit a
requisition to Purchasing for approval prior to placing the order. These types of purchases do not
comply with the current Purchasing Policy and as of October 1, 2013, Finance no longer
approves purchase order payments for services invoiced after-the-fact.
Staff is working with Purchasing and Finance to ensure requests/purchases are made according
to an approved strategy in accordance with the Purchasing Policy. Board approval to direct pay
the invoice is required.
OPTIONS: Approve/Disapprove.
RECOMMENDATIONS: Recommend approval for direct payment of the Airgas invoice.

SUBJECT: Approval of Quote for Stryker Stretcher (R1500214)
DEPARTMENT: Purchasing
BCC DISTRICT: All
STATEMENT OF ISSUE: [PUR] Seeking approval of quote for Stryker Stretcher (R1500214).
BACKGROUND & ANALYSIS: Public Safety entered a requisition to purchase four (4)
Stryker ambulance stretchers intended to replace aging inventory. Purchasing staff has received
quotes for the stretchers. Stryker provided the lowest quote at $51,828.96 ($12,957.24 ea.).
Purchasing policy requires that all purchases above $50,000 be purchased through a bid process.
However, the policy also states that the Board may approve purchases over $50,000 without
formal bids where it is deemed to be most advantageous to the County. Staff chose to use the
quote process for these items to avoid advertisement costs as well as expedite the purchase for
Public Safety. Staff requests Board approval of the lowest quote in lieu of bids for the purchase
of four (4) stretchers from Stryker for $51,828.96. Staff will process the requisition for a
purchase order if/when the Board approves the purchase. The stretchers were a budgeted item
with total funding at $52,400.
OPTIONS: Approve/Deny approval of the lowest quote in lieu of bids for the purchase of four

SUBJECT: Approval of Purchase of Vacuum Truck (R1500232)
DEPARTMENT: Purchasing
BCC DISTRICT: All
STATEMENT OF ISSUE: Seeking approval of purchase of a Vacuum Truck (R1500232).
BACKGROUND & ANALYSIS: The Water & Sewer Department budgeted $400,000 for a
sewer cleaning vacuum truck. Purchasing staff assisted Water & Sewer staff in researching state
cooperative purchasing opportunities for this purchase. These cooperative purchasing
agreements are competitively bid. They provide those utilizing their vendors the opportunity to
take advantage of their competitive pricing due to the volume needs. This vehicle has very
specialized specifications. It was to the County’s advantage to utilize bids already competitively
procured due to the specialized nature of the vehicle.
Staff has obtained quotes from the National Joint Powers Alliance (NJPA) and the Florida
Sheriff Association State Contract. The Florida Sheriff Association Contract price exceeded the
budgeted amount at $409,063. The NJPA, Sansom Equipment Company, provided the lowest
quote for a 2015 Freightliner at a total cost of $374,630 and staff has reviewed the specifications
and found they meet or exceed the needs of the department’s request. Staff requests Board
approval of utilizing the quotes from the cooperative agreements in lieu of a formal bid process.
Staff will process the requisition for a purchase order if/when the Board approves the purchase.
OPTIONS: Approve/Deny utilizing the lowest quote from NJPA, Sansom Equipment Company,
for the purchase of a 2015 Freightliner Vacuum truck at $374,630 for the Water & Sewer
Department.

DATE: December 2, 2014
TO: Honorable Chairman and Members of the Board
FROM: Jason Autrey
SUBJECT: Baker Landfill - 2015 Semiannual Sampling and Reporting
DEPARTMENT: Public Works
BCC DISTRICT:
STATEMENT OF ISSUE: Professional services are required for compliance with Florida
Department of Environmental Protection (FDEP) Permit/Certification #0075982-006-SF which
addresses long term care of the Baker Landfill. Professional services are also required to comply
with the FDEP Remedial Action Approval Order dated September 2, 2008.
BACKGROUND: Baker Landfill has two FDEP regulated environmental interests - Long Term
Care (LTC) and Corrective/Remedial Action. LTC is an FDEP permitted activity that requires
monitoring of groundwater from a designated network of sampling locations. The County has
been performing this compliance monitoring since the landfill was closed to land filling activities
in 1998 and was permitted for LTC status in 2003. Since that time, the County has complied
with the permit through sampling of the monitoring network.
The FDEP directed the County to initiate Corrective Actions in 1989 due to the detection of
chemical constituents in the groundwater exceeding FDEP water quality standards. The FDEP
approved the County’s Contamination Assessment Report in 1993 and the County began the
design of a Remedial Action Plan in (RAP) 1994. Several iterations of the RAP were produced
over the next few years and FDEP approved the County’s remediation plans in 2002. The
remedial action system became operational in 2004. Based on operational observations, the
remediation system has been slightly modified since becoming operational. After two years of
operation, the remediation system was shut down in September 2006 to address a compromise of
the liner system in one of the treatment ponds. The remediation system was not reactivated until
August 2008. Due to lack of maintenance and down-time, the remediation system was not
operating fully until May 2009. Current assessments of the operational characteristics of the
system indicate that the system is not performing to design specifications for a variety of reasons.
The most recent RAP Modification was approved by the FDEP in 2011 paving the way for
substantial infrastructure changes completed in 2013 which will ultimately result in substantial
reductions in the cost of restoration activities.
Both Long Term Care Semi-Annual Compliance and Remedial Action Assessment monitoring
are budgeted activities. The activities in this Task Order address services to be performed in
FY2015. In order to meet LTC Permit and Remedial Action requirements, the LTC and RAP
Mod requires professional services to execute two sampling/reporting events at a proposed price
not to exceed $75,600.
OPTIONS: Approve/Disapprove the Preble-Rish Task Order proposal (attached) for sampling
and reporting associated with Baker Landfill not to exceed $75,600. Preble-Rish is a Board
Packet Pg. 418
approved Engineering Services provider to the Public Works Department under contract C14-

STATEMENT OF ISSUE: Request Approval to pay outstanding charges incurred prior to
encumbering funds for the purchase of safety boots from Safety Source, Inc. In the amount of

until as recently as last year. After-the-fact meaning the department did not submit a requisition
to Purchasing for approval prior to allowing the boot truck vendor to come on premise and fit

Staff has worked with Purchasing to ensure future requests/purchases will be processed so
Finance can approve the payment with this vendor. Approval to pay the outstanding charges is

STATEMENT OF ISSUE: Special Forces Association, Chapter 7 is requesting a Public Event
Permit to raise funds for the families of the 7th Special Forces Group (Airborne).
BACKGROUND: The Special Forces Association holds an annual Jingle Bell Jog 10k / 5 k
running race to raise funds for the families of the 7th Special Forces Group (Airborne). Nine
hundred participants are expected. This year they have requested to hold the run along U.S.
Highway 98 from the entrance of The Boardwalk on Okaloosa Island to a turn around point
approximately one mile west of the Eglin AFB Beach facility.
This request is being coordinated with the Florida Highway Patrol (FHP), the Florida

SUBJECT: Request to advertise Oak Hill Road and Sexton Road Repairs
DEPARTMENT: Public Works
BCC DISTRICT: 1&3
STATEMENT OF ISSUE: Design plans for the construction and repair of both Oak Hill Road
(Crestview) and Sexton Road (Baker) are being finalized. As soon as final plans and quantities
are received, Public Works will develop the RFB package and submit to Purchasing for review.
Approval is being requested from the Board of County Commissioners to advertise both projects
for construction once the RFB package has been approved internally.
BACKGROUND AND ANALYSIS: In April 2014, a major overnight storm event caused
significant damage to much of Okaloosa County’s roadway and drainage infrastructure. Two of
the roadways most heavily damaged were Oak Hill Road in Crestview and Sexton Road in
Baker. The County tasked Preble-Rish, Inc. to perform the survey and to design the repairs for
each roadway.
The cost of these repairs will be 75% funded by direct reimbursement through FEMA’s declared
“State of Emergency” for this storm event. Other funding will include a 12.5% direct
reimbursement from the State of Florida leaving the County responsible for the remaining 12.5%
of the total construction cost. The total construction estimate submitted by Preble-Rish, Inc. for
each project is $713,938.25 (Oak Hill Road) and $622,236.03 (Sexton Road).
OPTIONS: Approve/Disapprove authorization to advertise the RFB for both the Oak Hill Road

FROM: Elliot Kampert
SUBJECT: Medicaid Amendment 8 and task order 28.8
DEPARTMENT: Growth Management
BCC DISTRICT: ALL
STATEMENT OF ISSUE: The Department of Growth Management requests Board approval
of Amendment No.8 to the agreement with the Commission for the Transportation
Disadvantaged (CTD) for reimbursement of Medicaid Non-Emergency Transportation (NET)
services and for Task Order No. 28.8 for reimbursement of operating costs to Okaloosa County
Transit (OCT).
BACKGROUND: The Board first approved the Commission for the Disadvantaged Medicaid
Non-Emergency Transportation Program Agreement on January 20, 2009.
Effective August 1, 2014 for Okaloosa County, the Florida Legislature established the Managed
Medical Assistance (MMA) program as part of the Statewide Medicaid Care program in Part IV
of Chapter 409, Florida Statutes. The MMA program requires Managed Care Organizations to
provide covered services to Medicaid recipients enrolled in managed care plans. Not all
Medicaid recipients are covered. The Commission for the Transportation Disadvantaged (CTD),
through its Subcontracted Transportation Providers (STPs), provides transportation to those
excluded from the MMA.
The attached Agreement is effective January 1 through February 28, 2015. The County will be
paid using a per-member per-month (PMPM) rate for categories listed in the agreement. No
Local Funds are required to receive Medicaid NET compensation from the CTD. Task Order
No. 28 amendment No. 8 will allow for the reimbursement of operating costs to OCT to provide
NET services for Okaloosa County.
RECOMMENDATION: The Department of Growth Management requests the Board to
approve Amendment No.8 to the agreement with the Commission for the Transportation
Disadvantaged (CTD) for reimbursement of Medicaid Non-Emergency Transportation (NET)
services, and sign Task Order No. 28 Amendment No. 8 for reimbursement of operating costs to

FROM: Sunil Harman
SUBJECT: Change Order No. 2 with Birkshire Johnstone, LLC., for West Hangar
Parking at CEW
DEPARTMENT: Airport
BCC DISTRICT: All
STATEMENT OF ISSUE: The Board of County Commissioners is requested to approve Change
Order No. 2 with Birkshire Johnstone, LLC to add contract time for select items and delete a work item
for the West Hangar Area Parking Facilities project at Bob Sikes Airport
BACKGROUND: Change Order No. 1 (approved April 17, 2014) added 63 calendar days to the
contract which established the final completion date as July 31, 2014. Several work items were not
complete (identified in Table 1 of Attachment A) at that time and this change order has been prepared to
provide additional contract time for Birkshire Johnstone, LLC to complete these items and deliver a
substantially completed and acceptable project. This time extension only applies to the work items
identified in Table 1. The terms and conditions of the original contract shall remain in full force and effect
and, by issuing this change order, the County does not waive any rights or remedies that it may have
under the original contract. These rights include a possible claim for liquidated damages for failing to
complete the work items not included in Table 1 within the contract time.
This change order also deletes $8,108.00 from Pay Item 43, Gulf Power Allowance. This allowance was
established as a mechanism to pay Gulf Power through the contractor to furnish and install the primary
power improvements. In an effort to complete the Gulf Power work as soon as possible, the County has
elected to pay Gulf Power directly and be reimbursed by the Florida Department of Transportation.
Several of Birkshire Johnstone’s subcontractors have notified the County of unpaid amounts due from
Birkshire Johnstone for work performed in July. This change order authorizes the County to remit
payment for Pay Application No. 8 directly to Birkshire Johnstone’s legal counsel to ensure payment is
provided to these subcontractors.
OPTIONS:
Approve, Reject or Table
RECOMMENDATIONS: It is Staff's recommendation that the Board approve Change Order
No. 2 with Birkshire Johnstone, LLC to add 140 calendar days for the items listed in Table 1 and delete
$8,108.00 from the contract.

BOARD OF COUNTY COMMISSIONERS
AGENDA REQUEST
DATE: December 2, 2014
TO: Honorable Chairman and Members of the Board
FROM: Don Turner
SUBJECT: New SO Building - Deductive Change Order 14-04
DEPARTMENT: Facility Maintenance
BCC DISTRICT: 2
STATEMENT OF ISSUE: Requests BCC approval of Deductive Change Order 14-04 to the
Lord & Son Contract on the Sheriff's New Building.
BACKGROUND: The Guaranteed Maximum Price (GMP) for the Sheriff's new building
included all the materials necessary to construct the building. In order to reduce the sales tax
cost on the project, the county is purchasing a large quantity of the materials directly through its
Owner Direct Purchase (ODP) program.
Since the materials cost, along with the sales tax, was included in the Construction Manager's
contract, a deductive change order is necessary. This will reduce the CM's contract by the
amount purchased through the ODP program including the sales tax on that amount. Over
$23,600 in sales tax has been saved as a result of the use of the ODP program on this project to
date.
Staff recommends approval of Deductive Change Order 14-04.
RECOMMENDATIONS: Request BCC approval of Deductive Change Order 14-04 in the
amount of $399,786.06 to the Lord & Son Contract on the Sheriff's New Building and for the
Board to authorize the Chairman to execute all related documents.

BOARD OF COUNTY COMMISSIONERS
AGENDA REQUEST
DATE: December 2, 2014
TO: Honorable Chairman and Members of the Board
FROM: Kay Godwin
SUBJECT: County Administrator Selection Process
DEPARTMENT: Human Resources
BCC DISTRICT: All
STATEMENT OF ISSUE: Board discussion of how to re-fill the position of County
Administrator.
BACKGROUND: Ordinance No. 93-17, passed by the Board of County Commissioners in May
1993 created the office of County Manager for Okaloosa County, Florida. The title later became
known as County Administrator under contract with James D. Curry entered into in July 2005.
The ordinance stipulates that the appointment of a County Manager shall be by an affirmative
vote of not less than three (3) members of the Board of County Commissioners. Beyond that,
there are no requirements as to the procedures that must be followed with respect to filling a
vacancy in the position of County Administrator.
Upon the resignation of Mr. Curry in 2013, the Board utilized the executive recruitment services
of Colin Baenziger & Associates (CB&A) who offered a warranty as part of their fixed fee of
$21,500. Their warranty stated, “If the selected individual leaves for any reason other than an
Act of God (such as total incapacitation or death) within [the] first year, CB&A will repeat the
search at no charge for our services. If he/she departs during the next year for any reason other
than an Act of God, we will repeat the search for the reimbursement of our expenses only.” Mr.
Ernie Padgett was hired in April 2013, and left in October 2014, past the first year, but within the
second year. Therefore, should the Board determine to utilize the services of CB&A, the County
would only be charged for their expenses in conducting the search. Mr. Baenziger has estimated
his expenses would be approximately $7500 to $9500, and would anticipate a time frame of 90
days to complete a new search.
Upon Mr. Padgett’s resignation effective October 1, 2014, the Board appointed John Hofstad to
serve as Interim County Administrator. His contract period is 90 days, with a provision for
extensions of time, if needed.
OPTIONS: Board discussion of how to re-fill the position of County Administrator. As
Ordinance 93-17 does not establish a required selection process, the Board has the authority to
utilize any approach that it believes would be appropriate. This could include consideration of
internal candidates for the position (current county employees); consideration of external
candidates for the position; utilizing the executive recruitment services of Colin Baenziger &
Associates as outlined in their contract warranty; or establishing an alternative selection process.
