STATEMENT OF ISSUE: Staff requests Board of County Commissioner (BCC) approval to
terminate the Lease Agreement with Hend-Rx Nutrition & Laboratories at 1310 D Oddfellow
Road, Fort Walton Beach, FL 32547. This unit is located in a building owned by Okaloosa
County Water & Sewer (OCWS), abutting the west side of the rear parking lot of the Water &
Sewer Administration Building.
BACKGROUND: OCWS purchased two (2) buildings on the east side of Oddfellow Road in
2006, and currently has five (5) tenants in these units whose annual payments currently total
$32,742.00 (includes sales tax). Staff has made notice to the public generally that space is
available via the OCWS website.
Lease #L15-0417-WS with Hend-Rx Nutrition & Laboratories commenced on 12/16/2014 and
terminates on 12/15/2015. A copy of the current lease is attached. Annual rent for this unit has
been $5,088.00, payable as 12- $424.00 monthly payments.
The Agreement states that the Lessee shall be entitled to extend the lease for two (2) additional
years. The Lessee has elected not to extend the lease. Therefore, Lease #L15-0417-WS needs to
be terminated/closed out. The Contract Close-Out Checklist is attached for your review.
December 1-15 will be prorated, with a payment of $212, to finish out the current term of the
lease.
OPTIONS: Approve or deny.
RECOMMENDATIONS: Staff recommends BCC approval to terminate Lease Agreement
#L15-0417-WS with Hend-Rx Nutrition & Laboratories.
