Inventory Memo Date: January, 2017 To: Mr. Jason Autrey, Director Public Works; Okaloosa County From: Brian Shepheard, Fixed Asset Delegate Property Custodian; Okaloosa County Subject: Solid Waste Recycling, Fixed Asset inventory; asset # 11844, Roll-Off Container Cover During the recent inventory conducted with Nick Lombardi we identified a situation that we request direction and acknowledgement from you on. The hydraulic cylinders that operate this Roll-Off Cover failed a while back. The truck 95G4348 that had the cylinders attached to it had been using a knuckle boom attachment. With the knuckle boom attached the cover was not able to be used. So with the cover not being needed the failed cylinders were not replaced. Months later the cylinders were removed and discarded as scrap. It was unknown at the time of removal by the fleet mechanic(s) that the roll-off cover was assigned a property number, so no documentation was submitted for the correct dispositioning of the failed cylinders. With your approval below we will request for purchasing to obtain board approval IAW the property manual policy for removal of this item from active inventory.
