AGENDA REQUEST
DATE: May 26, 2015
TO: Honorable Chairman and Members of the Board
FROM: Karen A. Chapman, MD., MPH. oiwmon Q) ¢ o geall
Director, Okaloosa County Health Department
SUBJECT: Approval of Resolution for FDOH — Okaloosa County Health Department Fee Schedule
DISTRICT: Al
REQUESTING DEPARTMENT: Health Department
STATEMENT OF ISSUE: Request Board approval for modification of FDOH — Okaloosa County
Health Department fee schedule which consists of a fee increase to birth/death certificates and select
environmental health fees.
BACKGROUND: The increase to birth/death certificates will align Okaloosa fees to those of similar
medium sized County Health Departments throughout the State.
The request for the increase to select environmental health fees will accommodate the increase in cost
incurred for conducting required regulatory activities. This increase will align with fees charged in
neighboring counties and restructure county fees to meet statutory requirements for environmental
health.
RECOMMENDATIONS: Board approval and signature of Chairman and Finance Director. A copy of
the signed fee schedule be returned to the Okaloosa County Health Department to the attention of
Laura Green, (850) 833-9070.
ENCLOSURE: Resolution for Okaloosa County Health Department Fee Schedule.
