AMENDMENT NO. ~ AGREEMENT between THE OKALOOSA COUNTY BOARD OF COUNTY COMMISSIONERS and OKALOOSA COUNTY COORDINATED TRANSPORTATION (dba Okaloosa County Transit) This Amendment made and entered into this __ day of September 2015 by and between OKALOOSA COUNTY, a political subdivision of the State of Florida having its principal office at 1804 Lewis Turner Blvd, Suite 206, Fort Walton Beach, FL 32547 (hereinafter referred to as "County") and OKALOOSA COUNTY COORDINATED TRANSPORTATION, a not-for- profit corporation having its principal office at 207 Hospital Drive, Fort Walton Beach, FL 32548 (hereinafter referred to as "OCT"). NOW, THEREFORE, in consideration for the mutual benefits contained herein, the County and OCT agree as follows: ATTACHMENT I - Agency/Operator Contract Service Description For the Period covering July ls\ 2015 - September 30, 2015 per rates established by the Commission for the Transportation Contract G0270 . Deleted in its entirety and amended to read as follows: A.
