FIRST AMENDMENT TO WORLD FIBER TECHNOLOGIES, INC. CONTRACT C10-1838-IS THIS AMENDMENT to the Agreement entered into by and between Okaloosa County, the owner, hereinafter referred to as the Party of the First Part, and World Fiber Technologies, Inc. or its successors, executors, administrators and assigns, hereinafter referred to as the Party of the Second Part, signed November 17,2010, is hereby amended as follows: First Amendment to Exhibit A (attached) to include additional items ALL OTHER TERMS OF THE ORIGINAL AGREEMENT SHALL REMAIN THE SAME. HilER OPTlC fEl"," OHK lAINTEN . \.~C EXPffiES : 1 111712012 Maintenance Contract for Okaloosa County Fiber Optic Network This Agreement, in triplicate, executed in Fort Walton Beach, Florida this J 7/J. day of tVev~•.J,U'- ,2010 is between Okaloosa County, Florida, the owner, hereinafter called the party of the First pati, and World Fiber Technologies, Inc. of its successors, executors, administrators and assigns, hereinafter called the Party of the Second pali. WITNESSETH: That for and in consideration of payments, hereinafter mentioned, to be made by party of the First Pali, the Party of the Second Pali agrees to furnish all equipment, machinery, tools and labor; to furnish and deliver all materials required to be furnished and delivered in and about two (2) years maintenance period to begin upon execution of signatures by both patiies, and to do and perform the maintenance of the Okaloosa County Fiber Optic Network and pay the unit prices as attached on Exhibit "A", which is made a part of this Agreement. Work to be issued on task orders as needed and in strict conformity with the provisions of this Contract. This Contract may be renewed for one additional two year period. SCOPE OF WORK: The project covers task orders for furnishing and installing 2" conduit, pull boxes, splice boxes, fiber optic cable, termination, splices, splice enclosures, emergency response and trouble shooting. All work on this contract is subject to the same standards as previous contracts for Phase 1 through phase V. The Contractor shall be prepared to begin work as required by the task orders. The place where the work is to be started will be stated in the task orders. The work shall be prosecuted from as many different points, in such part or patis and at such times as may be directed, and shall be conducted in such a malUler and with sufficient materials, equipment and labor as is considered necessary to insUl:e it s completion with the time set forth in the task orders. All specifications relating to conduit installation shall be compliant with what is set forth in the FOOT specifications for installation and the Technical Special provisions for Phase V. INSURANCE REQUIREMENTS: Contractor will provide a copy of insurance certificate listing Okaloosa County as additional insured party.
