STATEMENT OF ISSUE: Request approval for the Chairman to sign the Phillips Medical
Capital Delivery and Acceptance Certificate.
BACKGROUND & ANALYSIS: On January 19, 2016 the Board approved for the Chairman to
sign the lease documents for fifteen (15) cardiac monitor defibrillators for Public Safety. This
documentation authorized the equipment to be delivered to Public Safety for installation in the
ambulances. Purchasing has received confirmation from Public Safety that the cardiac monitor
defibrillators have been received and installed. To finalize the lease documents the Chairman’s
signature is required on the Delivery and Acceptance Certificate. This confirmation will also
allow the first payment of $8,504.97 to be submitted to Phillips Capital. Staff requests
authorization for the Chairman to sign the document confirming the delivery and installation of
the cardiac monitor defibrillators.
OPTIONS: Approve/Deny authorization for the Chairman to sign the Delivery and Acceptance
Certificate confirming the successful delivery and installation of the cardiac monitor
defibrillators.
RECOMMENDATION: Staff recommends approval for the Chairman to sign the Delivery and
Acceptance Certificate confirming the successful delivery and installation of the cardiac monitor
defibrillators.
