TWELFTH AMENDMENT
TO
TEEN COURT AGREEMENT
CONTRACT #C00-0417-BCC
THIS TWELFTH AMENDMENT to the Agreement entered into by and between
Okaloosa County, a political subdivision of the State of Florida, hereinafter referred to as
the “County,” and Lutheran Services Florida, Inc., hereinafter referred to as the “Teen
Court,” signed August 17, 2004, is hereby amended as follows:
Paragraph 1. TERM: The term of this amendment shall commence on January
1, 2016 and run through January 31, 2016 and thereafter shall renew on a month to month
basis until such a time as the County is fully equipped to render the services with internal
staff. Notification of the desire to extend or discontinue the Contract must be provided by
County to Teen Court in writing at least seven (7) business days before the end of the
month.
Paragraph 5. AGREEMENT AMOUNT: For services provided, the County
shall pay to the Teen Court, the sum of seven thousand, eighty-three and 00/100
($7,083.00), upon an appropriate invoice and approved in advance by the County.
ALL OTHER TERMS OF THE ORIGINAL AGREEMENT SHALL
REMAIN THE SAME.
IN WITNESS WHEREQF, this amendment to the Teen Court Agreement has been
executed by the County and Teen Court this day of 2015.
