BOARD OF COUNTY COMMISSIONERS
AGENDA REQUEST
DATE: July 15, 2014
TO: Honorable Chairman and Members of the Board
FROM: Dino J. Villani
SUBJECT: Outstanding Airgas Invoice for May 2014
DEPARTMENT: Public Safety
BCC DISTRICT: All
STATEMENT OF ISSUE: Request approval to pay Airgas, Inc., for the outstanding May 2014
invoice in the amount of $2,699.38.
BACKGROUND: Airgas, Inc., is the sole medical oxygen vendor for Okaloosa County
Emergency Medical Services. Historically, invoices for oxygen services have been paid by
after-the-fact purchase order. “After-the-fact” meaning the department did not submit a
requisition to Purchasing for approval prior to placing the order. These types of purchases do not
comply with the current Purchasing Policy and as of October 1, 2013, Finance no longer
approves purchase order payments for services invoiced after-the-fact.
Staff is working with Purchasing and Finance to ensure requests/purchases are made according
to an approved strategy in accordance with the Purchasing Policy. Board approval to direct pay
the outstanding invoice is required.
OPTIONS: Approve/Disapprove.
RECOMMENDATIONS: Recommend approval of direct payment for the outstanding invoice
to Airgas.

DEPARTMENT: Purchasing

STATEMENT OF ISSUE: [PUR] Request approval to enter into contract negotiations for
Dental Insurance, Life Insurance and Long Term Disability Insurance (RM RFP 44-14).
BACKGROUND & ANALYSIS: On February 4, 2014 Risk Management received approval
by the Board to solicit a Request for Proposals (RFP) to provide dental insurance, life insurance,
and long term disability (LTD) coverage. The RFP was advertised and on May 30, 2014
fourteen (14) proposals were received. The Review Committee met to discuss and evaluate the
proposals and has requested and received additional information regarding the submittals. The
review committee has unanimously voted Corporate Benefits of the Emerald Coast for Met Life
dental coverage as the most favorable submittal. The committee also gave high regard to Willis

contract negotiations with Corporate Benefits of the Emerald Coast. If a satisfactory agreement
cannot be reached they request approval of negotiation with Willis of Alabama for dental
coverage.

SUBJECT: Request to Advertise RFB for Tanglewood/Overbrook MOEX Grant
DEPARTMENT: Public Works
BCC DISTRICT: 4
STATEMENT OF ISSUE: Request for approval to advertise the Request for Bids (RFB) and to
administer the construction of Grant Agreement No. S0697 with the Florida Department of
Environmental Protection (FDEP) for improvements to the Tanglewood and Overbrook
Stormwater Basins. The construction of this project is being funded by an appropriation from

to be strictly utilized for project construction.
Final plans have been completed by the Northwest Florida Water Management District (WMD)
and have been submitted to Public Works. Once all permits are received, staff will advertise the
