THIRD AMENDMENT TO WORLD FIBER TECHNOLOGIES, INC. CONTRACT ClO- 1838-IS THIS A.MENDMENT has an effective date of_ _ _ _ _ _-', 2015 between the County ofOkaloosa, Florida and World Fiber Technologies, Inc. Contract C 10-18-38-IS will be expiring on March 17, 2015. Task orders have been assigned and that work assigned through the task orders will be ongoing and not completed before the contract expires. This contract is amended and extended to reflect that the assigned task order will be completed as written and paid at the rates so established under the original contract for which the task orders were written. The master contract will expire when the assigned task orders are completed with an anticipated completion date ofJuly 31,2015. Okaloosa County and World Fiber Technologies, Inc. desire and agree to amend the contract, in consideration of the mutual covenants herein and other good and valuable consideration. Okaloosa County and World Fiber Technologies, Inc. hereby agree as follows : Extend Term of Contract ClO-1838-IS until completion ofaB work assigned to the attached task orders. This applies to the following task orders: Task Order # 17 Anticipated completion date of July 31, 2015 Task Order # 18 Anticipated completion date of July 31 , 2015 Task Order #19 Anticipated completion date of July 31, 2015 Task Order #20 Anticipated completion date of July 31, 2015 This contract will terminate upon the completion of all work associated with these task orders or September 1, 2015, whichever is later. Ratification and Entirety. Except as provided in the Amendment, aU the terms and conditions of the Contract are hereby ratified and shall remain in full force and effect. This Amendment and the Contract, together will all supplements, exhibits and schedules thereto, shall embody the final, entire agreement among the parties hereto and supersede any and all prior commitments, agreements, representations and understandings, whether written or oral, relating the Contract for this Amendment or the subject matter hereof, and may not be contradicted or varied by evidence of the prior, contemporaneous or subsequent oral agreement between the parties hereto. Unless otherwise defined herein, all capitalized terms shall have the meaning set forth in the Contract.
