The School Health Services Plan is a two-year document designed to facilitate the establishment of local
strategies to implement activities mandated by law to meet the health of Florida's student and improve their
chances for success in school. Although the Department of Health in Okaloosa County takes the lead on
preparing this document, the law requires that this plan be collaboratively developed and approved by the county
health department (CHD), the local school district (LEA), contracted health provider (PSA) and the local school
health advisory committee (SHAC). The entities involved have met and have been a part of the development of
this document.
