FIRST AMENDMENT TO THE CORIZON HEALTH INC. AGREEMENT
THIS FIRST AMENDMENT TO THE INMATE HEALTH SERVICES
AGREEMENT (hereinafter the “Amendment”), is made and entered into this day of
July 2014, by and between Corizon Health, Inc., (hereinafter “Corizon”) and the Okaloosa
County Board of County Commissioners on behalf of the Okaloosa County Department of
Corrections (hereinafter the “County”).
WHEREAS, the County and Corizon entered into an Inmate Health Services Agreement
on August 15, 2013 (hereinafter the “Agreement”), by which Corizon assumed the
responsibilities for the provision of certain health care services to be delivered to individuals in
the custody and control of the County; and
WHEREAS, Corizon and the County desire to amend the Agreement again to effectuate
the following changes:
NOW, THEREFORE, in consideration of the mutual covenants contained herein, the
receipt and sufficiency of which are hereby acknowledged, it is mutually agreed upon as follows:
ile The County employs a Registered Nurse and Corizon provides an employee with
qualifications to fulfill the duties of the County Nurse to backfill the position when
the County Nurse takes any time off of work in excess of three (3) consecutive
working days. The County agrees to reimburse Corizon for the cost of covering the
County Nurse position at an hourly rate of $36.11. This rate will be adjusted
annually at the same rate of any Consumer Price Index adjustment to the Agreement.
The cost of the backfilled hours will be added to the monthly invoice as appropriate.
Ph Upon termination of employment of the County Nurse, Corizon will adjust its current
staffing plan to include an additional 1 FTE RN. The parties will negotiate a
mutually agreeable rate for that position at that time.
3; In all other respects, the terms and conditions of the Agreement, as amended, shall
continue unchanged and in full force and effect.
IN WITNESS WHEREOF, the Parties hereto have executed this Amendment (First Amendment)
in their official capacity and with legal authority to do so.
