BOARD OF COUNTY COMMISSIONERS AGENDA REQUEST DATE: December 1, 2015 TO: Honorable Chairman and Members of the Board FROM: Jason Autrey SUBJECT: Tanglewood/Overbrook Stormwater Improvements - Change Order 5 Approval DEPARTMENT: Public Works BCC DISTRICT: 4 STATEMENT OF ISSUE: Public Works requests Board approval of a change order for the Overbrook/Tanglewood Stormwater Improvements Project. The change order allows the Contractor to modify the spillway design by adding additional concrete aprons to the backside of the spillway to prevent erosion and washouts. This change order requires Board approval due to the overall additional time that has been added to the contract. BACKGROUND: As a result of a settlement related to the Deep Water Horizon Oil Spill, FDEP allocated funds to Okaloosa County to assist with stormwater improvements in the Gap Creek Watershed Area. The two projects identified were the Tanglewood Stormwater Basin and the Overbrook retention area in Fort Walton Beach. Substantial completion for all related work under this contract was completed on October 20, 2015. During a final inspection with County staff, the Contractor and FDEP, it was discovered that the downstream side of the new spillway at Tanglewood Pond had sustained erosion from a recent heavy rain event. It was agreed by all present to utilize the remaining grant funds via change order and allow the Contractor to form and pour additional concrete slope protection along the main flume. The additional cost for this work is $4,198.00 which is within the current funding allocations of the grant agreement. The additional time needed to complete the work from the prior substantial completion date is 51 days resulting in a final contract date of Dec. 10, 2015. At the completion of this work, an additional final inspection will be made and project closeout will commence. Final payment is anticipated to be made prior to the end of the year.
