BOARD OF COUNTY COMMISSIONERS AGENDA REQUEST DATE: September 15, 2015 TO: Honorable Chairman and Members of the Board FROM: Zan Fedorak SUBJECT: Request approval of contract amendment 2 with Asphalt Paving Systems DEPARTMENT: Purchasing BCC DISTRICT: All STATEMENT OF ISSUE: Request approval of the Contract Amendment 2 with Asphalt Paving Systems, Inc to extend the expiration date of the contract to allow for the completion of all open task orders. BACKGROUND & ANALYSIS: On June 27, 2012, Okaloosa County entered into an agreement with Asphalt Paving Systems, Inc to provide micro-surfacing materials and labor. It has been determined that there are open task orders that cannot be completed by the expiration date of September 30, 2015. Therefore staff is requesting an extension of the contract. These task orders include road lists for micro surfacing previously approved by the Board. OPTIONS: Approve/Deny the request of Contract Amendment 2 with Asphalt Paving Systems, Inc to extend the expiration date of the contract to allow for the completion of all open task orders and authorize the Chairman to sign the contract amendment. RECOMMENDATION: Staff recommends approval of Contract Amendment 2 with Asphalt Paving Systems, Inc to extend the expiration date of the contract to allow the completion of all open task orders and authorize the Chairman to sign the document.
