Department of Growth Management State of Florida TO: Zan Fedorak, Purchasing Manager FROM: Elliot Kampert, Growth Management Director RE; Inventory Item — Cable Box Date: June 17,2016 Inventory reconciliation was conducted on Transit Assets on February 23, 2016. At that time all items were accounted for except a satellite TV box, asset #16488. The box was originally purchased in 2006 for $1,207.00. In 2015, the box stopped working and contracted employees contacted the satellite company for repair. The service person was unable to repair the box and replaced it. At that time, it is believed that the service person took the box and was unaware that the box was not a rental but rather County property. Growth Management therefore believes the box was inadvertently disposed of and not stolen. The asset has a depreciation value of zero. All staff have been advised to contact the department delegate property custodian on any changes to property accountability in each facility. Growth Management requests for removal of item from active status.
