BOARD OF COUNTY COMMISSIONERS AGENDA REQUEST DATE: November 3, 2015 TO: Honorable Chairman and Members of the Board FROM: Jeff Littrell SUBJECT: Final Payment - Test America Contract DEPARTMENT: Water and Sewer BCC DISTRICT: All STATEMENT OF ISSUE: Okaloosa County Water & Sewer (OCWS) staff requests Board of County Commissioners (BCC) approval of a final payment, in the amount of $206.25, to Test America on contract #C95-0124-WS. BACKGROUND: Via the bidding process, OCWS entered into a new contract (#C15-2324- WS) with Test America effective August 18, 2015. This contract is for laboratory testing and analysis that go beyond Okaloosa County Water & Sewer’s in-house laboratory's scope and abilities. The previous contract (#C95-0124-WS), which was also with Test America, expired September 30, 2015. The attached invoice is dated September 30, 2015 for work performed in August and represents the final invoice and payment on this previous contact. The final payment on this contract is now due. This final invoice, along with other pertinent information, is attached for your review. OPTIONS: Approve or deny. RECOMMENDATIONS: Staff recommends BCC approval of a final payment, in the amount of $206.25, to Test America on contract #C95-0124-WS. RECOMMENDED BY: APPROVED BY: John Hofstad, County Administrator
