BOARD OF COUNTY COMMISSIONERS AGENDA REQUEST DATE: January 5, 2016 TO: Honorable Chairman and Members of the Board FROM: Jason Autrey SUBJECT: Final Pay Request for Asphalt Paving Systems, Inc. C12-1961-PW DEPARTMENT: Public Works BCC DISTRICT: 2, 4, & 5 STATEMENT OF ISSUE: Public Works provides a list of roadways to receive surface treatments annually to the Board of County Commissioners for consideration and approval. Task order 3 for contract C12-1961-PW was presented in February of 2015 and subsequently approved. The scope of work authorized as part of the contract has been completed and approved; this is the final pay request for task order 3 for contract C12-1961-PW and satisfies the requirements for C12-1961-PW. BACKGROUND: APS has been contracted by Okaloosa County to provide pavement preservation alternatives since 2012 and have exhausted all available extensions. All work associated with this contract and all assigned task orders are complete and authorization to make final payment is requested. Future work of this type will be performed under a new contract procured in accordance with County Purchasing policies, which Public Works is currently working on. OPTIONS: Approve or Disapprove final payment to Asphalt Paving Systems, Inc. in the amount of $194,626.90 and authorize the closing of contract C12-1961-PW.
