The activities that are included are described in "Scope of Services." SCOPE OF SERVICES The CONTRACTOR (World Fiber Technologies, Inc.) agrees to perform the technical and construction related services in accordance with contract C 10­ 1838-IS dated November 17 th , 2010 as requested and directed by the OWNER. 1. Furnish and installl 2" conduit system from PB9242 to PB 1856 by HDD on McWhorter st. 2. Install OWNER provided pull box. 3. Install 2 concrete collars. Installation of concrete collars will be billed at the rate of 3 conduit/fiber repair crew hours each. 4. Install OWNER provided 48 count SM Corning FOC from PB9242 to PB 1660. 5. Provide and/or buj,/d out necessary splice enclosures and fusion splice according to OWNER provided splice diagrams. 2  Okaloosa County Information Systems Fiber Optic Maintenance Contract Number C 10-1838-15 Task Order 18 (McWhorter st) COM PENSATION PROVISIONS As compensation for providing the services under this Task Order, the OWNER shall pay the CONTRACTOR in accordance with the November 17th, 2010 agreement for the actual work completed and acce pted by the OWNER. Invoicing must show specific quantities and grouped by each assigned task with sufficient detail to fully report results, condusions, and recommendations as required . Each invoice will also show total to date of all invoices under this task order. The Not to Exceed (NTE) amount for this task order is: __$16,000.00_ __ PERIOD OF SERVICE The schedule for the services provided under this task order shall be in accordance with the November 17, 2010 Agreement and will rema:n in effect unHI the scope of work is complete or until the NTE amount is reached or otherwise terminated by the OWNER.
