Services will be used on an as needed basis by all schools, PTO, athletic boosters, band boosters, and other support organizations that provide direct support for student activities. The contract is effective September 9, 2014 through June 30, 2015. A copy of the agreement is attached. The amendment adds a fee schedule for classes that exceed fifty (50) minutes in length. The fee schedule is as follows: 51-90 minutes @ $5.25 per student per lesson (max) 91-120 minutes @ $6.50 per student per lesson (max) All other terms and conditions of the original service agreement shall remain in full force and effect. The Provider will conduct jazz/hip hop clinics, providing choreography for the nationals routine, music production, costuming assistance and completing the necessary paperwork for the competition. The Provider will be paid $160.00 per hour for a maximum of twenty-five (25) hours, for a total contract amount not to exceed $4,000.00. Funds will be paid form internal funds, Eaglettes Dance Team. The contract is effective September 12, 2014 through September 14, 2014. A copy of the agreement is attached.
