The School Health Services Plan is a two-year document designed to
facilitate the establishment of local strategies to implement activities
mandated by law to meet the health of Florida's students and improve their
chances for success in school. Although the county health department
(CHD) takes the lead on preparing this document, the law requires that this
plan be collaboratively developed and approved by the county health
department (CHD), the local school district (LEA), contracted health
provider (PSA) and the local school health advisory committee (SHAC) and
to include a process for data collection that can be evaluated. The entities
involved have met and have been part of the development of this document.

Recommended Action : Motion to approve the
Campbell Grant Application
for Elliott Point Elementary
School, in the amount of
$30,000.00.

Mr. Don Campbell is a private donor who enjoys helping others through
financial support. Elliott Point Elementary has been connected with Mr.
Campbell through a personal relationship with a stakeholder.
Mr. Campbell shared his process for consideration- "The way my foundation
works is that people submit proposals usually from organizations and then I
act on them. If I decide I want to fund the proposal then I ask my Board for
approval which I usually receive. Before sending you the information on
how to submit a proposal, I would like to hear a little bit about what your
proposal would cover so that I can decide if it you should submit a proposal.
If necessary I would be willing to talk to you about it on the phone. I wait to
hear from you."
After speaking with Mr. Campbell and sharing the demographics and needs
of Elliott Point, he had his attorney, Ms. Nancy Kuhn, send us the grant
application. Ms. Kuhn informed us that Mr. Campbell was prepared to
entertain a proposal for $30,000.00 to assist our school.

lower grades. Dustin Keith assisted us with deciding on iPads, cases,
keyboards, mice, and charging stations. iPads were chosen over other forms
of technology due to no recurring costs to keep them in use. Grades K-2
were chosen to receive 5 iPads per classroom. At this time, a lack of
technology is a major concern at Elliott Point. Grades K-2 were chosen to
increase digital literacy at a young age. Currently, technology is slim in
these classrooms. The equipment cost is just shy of $30,000.
I have had Mr. McInnis review the proposal and also the attorney's
statement of expectations of the support. Ms. Kuhn provided this response -
"we require a report verifying that the funds were spent for educational
and/or charitable purposes. The report is due within one year of the grant
award date. The Board of the
