BOARD OF COUNTY COMMISSIONERS AGENDA REQUEST DATE: October 20, 2015 TO: Honorable Chairman and Members of the Board FROM: Jason Autrey SUBJECT: Final Payment - Jail Parking Lot Repair DEPARTMENT: Public Works BCC DISTRICT: 1 STATEMENT OF ISSUE: Northwest Florida Land Clearing, Inc. is requesting final payment, including release of retainage, for contracted work associated with the Okaloosa County Jail Parking Lot Repairs Project (FEMA). BACKGROUND AND ANALYSIS: On February 4, 2015 Okaloosa County entered into contract with Northwest Florida Land Clearing, Inc. to perform the work associated with the County jail parking lot repairs from the April 2014 storm. All work has been completed and found to be in compliance with the approved plans and specifications. Substantial Completion was achieved on May 15, 2015 and Final Acceptance of the project was issued on June 12, 2015 after a final inspection was conducted by County staff. The Contractor has submitted the final invoice and closeout documentation and is requesting final payment and release of retainage. Through the course of the project, there was one change order resulting in a net increase of $3,462.50 (5%) with 5 rain days added to the contract time. A final pay application for the contract balance of $3,579.08 is attached. This final pay request will release all retainage and cause the work to enter the one year performance warranty period. OPTIONS: Approve/Disapprove final payment to Northwest Florida Land Clearing, Inc. in the amount of $3,579.08 for work associated with the Okaloosa County Jail Parking Lot Repairs (FEMA). RECOMMENDATION: Approve final payment to Northwest Florida Land Clearing, Inc. in the amount of $3,579.08 for work associated with the Okaloosa County Jail Parking Lot Repairs (FEMA).
