FOR I MID COUNTY TANK #4 I NWFWMD CONTRACT NUMBER 14-061 I WITNESSETH: THIS AGREEMENT AMENDMENT is made and entered into this _ _ day of , 2015, by and between the NORTHWEST FLORIDA WATER MANAGEMENT DISTRICT, a public entity created by Chapter 373, Florida Statutes,. as amended, for itself, herein after referred to as the "DISTRICT," and OKALOOSA COUNTY, hereinafter referred to as the "GRANTEE." In consideration of services to be performed for the public benefit hereunder, the DISTRICT and the GRANTEE hereby agree to amend DISTRICT CONTRACT Number 14-061 referenced above as follows: I. Paragraph 3.A. is revised to read as follows: "3.A. This AGREEMENT shall remain in effect until September 30, 2017, unless extended by mutual written consent." 2. Attachment 1 - Background is revised to read as follows: The project will provide a new seven hundred fifty thousand gallon elevated water storage tank to provide additional elevated storage to facilitate maintaining pressure to customers in the northern Mid County service area when the existing tank is taken out of service for maintenance and will provide additional storage for water that is sent to the south County, as required by the Regional Water Supply Plan to reduce pumping from coastal wells. 3. 4. Attachment 1 - Project Description is revised to read as follows: The project will consist of the construction of a new seven hundred fifty thousand gallon elevated storage tank. Attachment I - Anticipated Schedule is revised to read as follows: I Task Scheduled Completion Date Land Acquisition, Surveying Months 1-11 Design, Permitting Months 12-21 Construction Months 22-41 5.
