BOARD OF COUNTY COMMISSIONERS AGENDA REQUEST DATE: April 21, 2015 TO: Honorable Chairman and Members of the Board FROM: James Puckett SUBJECT: Lease - Former FWB Hospital - Opportunity Inc DEPARTMENT: Facility Maintenance BCC DISTRICT: 2 STATEMENT OF ISSUE: Consideration of an amended 10 year lease with the Okaloosa Walton Homeless Continuum of Care, Opportunity Inc. for space in the former Ft. Walton Beach Hospital. The annual fee shall be one dollar ($1.00). BACKGROUND: On October 7, 2014 the Board of County Commissioners approved the space allocation for both the Council on Aging (Elder Services) and Okaloosa Walton Homeless Continuum of Care, Opportunity Inc, within the old Ft. Walton Beach Hospital and directed staff to prepare leases that reflect the terms and conditions for the use of the approved space by the respective lessees. The County Attorney's office prepared the amended lease (attached) for Opportunity Inc. of which Exhibit "A" depicts the boundary of the leased area. Lease Area "B" is where the lessee has occupancy rights and the Common Area Maintenance "B" is where the lessee has exclusive maintenance (custodial services, carpet cleaning, and decorating) responsibilities. As this facility lease adjoins the lease with Elder Services a single, dimensioned floor plan depicting the lease area is included with each document. As directed by the board, staff facilitated an agreement regarding the portion of utilities bills for which Opportunity Inc would reimburse Elder Services; this agreement is included as exhibit "B" to the lease. This agreement will be reviewed annually in October and the amount shall be adjusted to reflect any changes in these costs if warranted. Terms of the lease have been approved by representatives of Opportunity, Inc and the contract has been routed through the proper internal departments (legal and risk management) to verify that documentation is in order. OPTIONS: Approve/Disapprove the proposed lease with Opportunity, Inc. to utilize space in the former Ft. Walton Beach Hospital. RECOMMENDATIONS: Approve an amended 10 year lease with Opportunity, Inc for space in the former Ft. Walton Beach Hospital as indicated on Exhibit "A". The lease areas included are identified as Lease Area "B" along with Common Area Maintenance "B" as indicated on Exhibit "A" at an annual lease rate of $1.00/year and for the Board to authorize the Chairman to execute all necessary documents. RECOMMENDED BY: APPROVED BY: John Hofstad, County Administrator
