The Benefits Oversight Group has reviewed all materials presented by FBMC Benefits Management related to the renewal of the
medical, dental, life, vision and long-term disability plans and voted to continue each with the incumbent carrier. A program summary and
individual renewal exhibits are attached:
Employee Benefit Renewal Summary
United Healthcare Insurance Renewal Exhibit
Delta Dental Insurance Renewal Letter (Carries over from 2019 Plan Year)
EyeMed Vision Insurance Renewal Exhibit - High Plan (Carries over from 2019 Plan Year)
EyeMed Vision Insurance Renewal Exhibit - Low Plan (Carries over from 2019 Plan Year)

Flexible Spending Account (FSA) Renewal Letter
FSA Agreement from 2016 (for reference only)
The Group Medical Plan through United Healthcare was the only plan for which there will be an increase - 5.5% - for the 2020 Plan Year.
Each of the other plans will continue with no increase due to rate guarantees included in previous renewals. There were no plan design
or other changes to any of the plans.
The FSA and HRA Agreements administered by Lockard & Williams Insurance Services, P.A. are also presented
for approval of one year, annually renewable extensions of the original agreements (attached for reference).
Execution of the attached letters will extend the agreements for the 2020 plan year per Section 2 TERM of the
original agreements.
The annual employee maximum allowable contribution limit for the FSA plan has been increased from $2,650 to $2,700 in accordance
with the most recently enacted changes to the Internal Revenue Code (IRC) Section 125(i). Recall that, during the 2018 renewal cycle,
the Certificate of Adopting Resolution and Summary of Material Modifications documents were modified to set the contribution limit at the
maximum allowable under IRC Section 125(i).
