above) for preparation of equipment blanks. To collect the equipment blank, the laboratory deionized water will be poured through the Nalgene sampler and into a pre-cleaned laboratory provided bottle. The equipment blank will be shipped along with the field samples to the laboratory. Equipment blanks will also be collected from the dipper if used for grab sampling after the dipper has been decontaminated. 3.3 Laboratory Control Sample The LCS is a clean-matrix QC sample that is spiked with a known amount of target compounds. The LCS is carried through the analytical process with the field samples. Its purpose is to monitor the effectiveness and efficiency of the analytical process at recovering target compounds. 3.4 Corrective actions in the event of QC problems If QC results indicate problems with the data, corrective action will be taken, which can include any of the below processes:  Retrieving missing information  Re-calibrating the measurement system  Re-analyzing samples (within holding time requirements)  Collecting additional samples  Qualifying results. June 2016 | 15 Gap Creek Stormwater Improvements Quality Assurance Project Plan Okaloosa County, Florida 4 Data Management Procedures 4.1 Sample Collection Records The recording and reporting of results of these monitoring events shall be in accordance with the specifications in Attachment J, Quality Assurance Requirements, of the Agreement, in Section 4. 4.2 Laboratory Data The data package from the laboratory will include a case narrative discussing any problems with the analyses, corrective actions taken, changes to the referenced method, and an explanation of data qualifiers. QC results associated with the data will also be included in the lab data package. The laboratory will provide all QC sample results and raw data in addition to the field sample results. This data package will be submitted in electronic format for review. All QC data will be included in the data report. Any errors with quality control will be noted in the data package. If the results do not fall within established control limits, the measurement system should be re-calibrated. The following minimum qualifications will be made, as necessary, by the laboratory:  U = Analyte is not detected  J = Analyte concentration less than the QL listed in Table 2 but above the DL. The J-qualifier may also address additional QC deficiencies which will be described in the laboratory case narrative. 4.3 Data Verification and Validation 4.3.1 Data Verification All analytical data generated during this study will be reviewed by the Laboratory Quality Assurance Manager (QAM). The QAM will ensure that:  Data are consistent, correct, and complete, with no errors or omissions  Results from the lab and field QC samples accompany the results  Established criteria for QC results are met  Data qualifiers are properly assigned where necessary  Data specified in the Sampling Process Design were obtained  Measurement procedures used to generate the data are those specified in this QAPP 4.3.2 Data Validation Data will be validated by HDR for the PARCCS parameters. Data will be qualified during data validation when PARCCS parameter MQOs as specified in Section 2.4 are not met. Data will be validated according to the USEPA Functional Guidelines (August 2014), and 16 |June 2016 Gap Creek Stormwater Improvements Quality Assurance Project Plan Okaloosa County, Florida final qualifiers will be R (rejected), J (estimated), U (not detected) and UJ (not detected, with an estimated reporting limit). Also, 100 percent of the data in the data tables that will be uploaded into STORET will be checked for accuracy against the data reported in the data package. 4.4 Data Upload As per grant requirements, monitoring results will be uploaded to the USEPA STORET database or the successor database to STORET by HDR. Data will be maintained by HDR in an electronic and hard copy form for a period of no less then three years following the final report. Data will be flagged following laboratory and HDR QC evaluations. 4.5 Data Quality Assessment If MQOs have not been met for data, HDR will determine if data is still usable. If data are found to be inadequate to meet study objectives, corrective action may be taken including, but not limited to:  Notifying the laboratory to re-analyze samples (if within the holding time requirements)  Re-collecting samples  Altering the sample collection method  Altering the measurement method 4.6 Audits and Reports The laboratories, in order to obtain their certifications with the Florida Department of Health, have been audited and accredited that they are technically competent and able to generate data of known and documented quality. It is therefore not anticipated that the labs will need to be further audited, nor will they need to analyze additional quality control check samples (QCCS) as indicated in Addendum 1, Quality Control Requirements for Laboratories Performing Chemical Analyses, of the Agreement. 4.7 Field Monitoring Report Monitoring results will be uploaded to the USEPA STORET database or the successor database to STORET. Through the STEPL program, an estimate reduction load for sediment, (TSS as estimated through the PLOAD model) will be reported. The following deliverables will be provided to gauge the effectiveness of the BMP  Four quarterly reports of the sampling results  Analysis of collected data with a Draft and Final Project Report  Records of data uploaded to the EPA STORET database.
