Villani SUBJECT: Outstanding Airgas Invoice Payments DEPARTMENT: Public Safety BCC DISTRICT: All STATEMENT OF ISSUE: Request approval to pay Airgas, Inc for two outstanding invoices (September 2013 for $2770.50 and November 2013 for $3611.66) for a total of $6382.16. BACKGROUND: Airgas, Inc is the sole medical oxygen vendor in our area and is used by EMS. Historically, invoices for oxygen services have been paid by after-the-fact purchase order. “After the fact” meaning the department did not submit a requisition to Purchasing for approval prior to placing the order. Finance no longer approves purchase order payments for services invoiced after-the-fact. Staff has worked with Purchasing to ensure future requests/purchases will be processed so that Finance can approve the payment with this vendor. Approval to pay the outstanding invoices is requested so that we can continue to purchase oxygen from this vendor. OPTIONS: Approve/Disapprove. RECOMMENDATIONS: Recommend approval of a direct payment for the outstanding invoice to Airgas. BACKGROUND: An annual maintenance contract with a two year initial contract period for the rehabilitation of manholes and lift station wet wells was awarded on October 1, 2013. The initial phase, consisting of 13 manholes, is nearly complete. OCWS staff is satisfied with the quality of work performed on the original phase and is ready to add to the contract, to facilitate the rehab of at least 25 more existing manholes and the new Emerald Breeze Lift Station wet well (on Okaloosa Island) before it is placed in service. The contract was awarded to Gulf Coast Underground, LLC. of Mobile, AL in the amount of $46,195.00 in Oct 2013. This change order will add $74,835.00 to the original contract, for a revised contract total of $121,030.00. Although this is called a "change order", no changes are being made to the contract terms and conditions, time, or unit prices; this change order simply adds quantities, as was originally intended to be Phase II. OPTIONS: Approve or deny. RECOMMENDATION: OCWS requests BCC approval to authorize the Chairman to execute Change Order # 1 to Contract # C13-2006-WS, adding $74,835.00 in scope of work to Gulf Coast Underground.
