RESOLUTION NO. 76-0 A RESOLUTION AUTHORIZING THE CITY MANAGER ON BEHALF OF THE CITY OF FORT WALTON BEACH TO ENTER INTO ENGINEER SERVICE CONTRACTS FOR PUBLIC WORKS PROJECTS WITH J. B. CONVERSE AND COMPANY, INC., PROVIDING THE EFFECTIVE DATE HEREOF. NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF FORT WALTON BEACH, FLORIDA. SECTION I. The City Manager on behalf of the City of Fort Walton Beach, Florida, is hereby authorized to enter into the attached contracts with J. B. Converse and Company, Inc. for engineering services on the following projects: Improve sewer lines as outlined in Local Public Works Capital Development and Investment Project Application FWB-7. Improve drainage system as outlined in Local Public Works Capital Development and Investment Project Application FWB-8. Drill and complete four shallow water wells as out- lined in Local Public Works Capital Development and Investment Project Application FWB-6 Extend and replace water lines as outlined in Local Public Works Capital Development and Investment Project Application FWB-9 SECTION II. This resolution being of an administrative nature shall take full force and effect immediately upon its passage and approval by the Mayor. ADOPTED Fo pro 7/7 76 Lh NE. SMITH City Attorney o) Oo AMENDMENT NUMBER 2 TO CONTRACT DATED MARCH 15, 1972 BETWEEN THE CITY OF FORT WALTON BEACH, FLORIDA AND J. B. CONVERSE & CO. , INC. The above Contract is amended in the following particulars only and the provisions herein apply only to work covered under this Amendment Number 2: The work covered by this Amendment is included in the applications for federal funds, under provisions of the Local Public Works Capital Development and Investment Program as authorized under PL 94-369, for the following projects: 1. Install and connect four (4) new wells to the Golf Course irrigation system; construct new potable water transmission main from elevated tank in Golf Course Area to existing city water main on Denton Blvd; and connect the existing well No. 10 to this water main. 2. Construction of various storm drainage facilities. as outlined in the Storm Drainage Grant Application. 3. Construction of various sanitary sewer improvements as outlined in the Sanitary Sewer Improvement Grant Application. 4. Construction of miscellaneous water system improvements as outlined in the Water System Improvement Grant Application. O oO B. Delete Article II, Basis of Payment, in its entirety and in lieu thereof substitute the following: Basis of Payment 1. Design Phase The Basis of Payment for design engineering services ‘under this Amendment No. 2 will be on the Lump Sum basis for each of the projects outlined in A. above. Said services as described in the grant applications to EDA under the Public Works Capital Development Program. The Lump | Sum design service for each project is as follows: Project No. 1 - $24, 600. 00 Project No. 2 - $14, 300. 00 90% of the design for this project has been completed by the City. The Engineer will prepare construction specifications, complete the project design and drawings, and assemble all documents for bidding purposes. Project No. 3 - $24, 800. 00 Project No. 4 - $32, 250. 00 2. Construction Phase - All Projects Payment to the Engineer for this phase of the work will be at the hourly rate shown on Exhibit A attached hereto times a multiplier of 2.30 for the personnel actually engaged in the work. Accurate time sheets certified by the Engineer shall be kept. Cost of travel, subsistence, telephone, telegraph, prints and printing and general out of pocket expenses required specifically for this project will be paid at actual cost plus handling charges of 10%. O : Oo IN WITNESS WHEREOF, this instrument is executed in duplicate, one copy being held by each party, this 10th day of November , 1976.
