STATEMENT OF ISSUE: Staff requests an amendment for contract C12-1999-BCC and
authorization for the Purchasing Manager to sign the document.
BACKGROUND: On November 5, 2012, the Okaloosa Board of County Commissioners
entered into a contract with Lewis Funeral Home, Inc. for indigent/unclaimed cremation services
for Okaloosa County. This contract was valid for three years with an extension option for three
additional years.
Florida law requires all counties to be charged with the burial of unclaimed or indigent bodies at
the county’s expense. The provisions of chapter 406, Florida Statutes, authorize the adoption of
procedures by a County for the disposition of the bodies of deceased indigent persons and
deceased unclaimed persons. At this time, staff has reviewed the procedures currently utilized
by the County regarding the disposition of unclaimed and indigent persons and has crafted a
draft uniform policy and procedure for disposition of the bodies of deceased indigent and
unclaimed persons to bring before the Board for approval at a future date. As a result of these
changes, the County is in the process of seeking competitive solicitations for indigent and
unclaimed cremation services.
To continue with this process, staff requests to amend the current contract with the following
provisions: The contract shall be extended on a month-to-month basis and shall be effective
retroactive to October 1, 2015. The contract amount is within the Purchasing Manager's signature
authority. Staff requests Board authorization for the Purchasing Manager to sign the
amendment.
OPTIONS: Approve/Disapprove the amendment for contract C12-1999-BCC and authorize the
Purchasing Manager to sign the amendment.
RECOMMENDATIONS: Staff recommends the Board approve the amendment for contract
C12-1999-BCC and authorize the Purchasing Manager to sign the amendment.
