WITNESSETH: That for and in consideration of payments, hereinafter mentioned, to be made by the Party of the First Part, the Party of the Second Part agrees to provide Life Insurance, Accidental Death and Dismemberment Insurance (AD&D) and Long Term Disability Insurance (LTD) in strict conformity with the provisions of this Contract, and the Contractor’s response to RFP #44-14 (the “‘Proposal’”’) approved by the Owner and attached hereto as Exhibit A. Eligible retirees may continue to participate in the group term life insurance plan as outlined in Exhibit B attached hereto. The said Proposal, including the form of Life, AD&D, and LTD policies attached thereto that will be issued to the Owner is hereby made a part of this agreement as fully and to the same effect as if the same had been set forth at length in the body of this agreement. In consideration of the foregoing promises, the Party of the First Part agrees to pay to the Party of the Second Part such unit prices for the insurance as set out in the Proposal in the manner provided in the Proposal, and in Exhibit B attached hereto, beginning October 1, 2014. This contract shall be for a three (3) year period beginning October 1, 2014 and running through September 30, 2017 at the rates stated in the Proposal. Further, the contract maybe renewed for two (2) additional one (1) year terms at a rate agreed upon by both parties. Renewal notice is to be provided to County of Okaloosa, Florida at least ninety (90) calendar days prior to the Anniversary Date.
