SUBJECT: Gum Creek Farms - Old Antioch Rd. Closeout

STATEMENT OF ISSUE: Gum Creek Farms, Inc. contracted with Okaloosa County to
perform the work associated with the Old Antioch Road Improvements Project. All work has
been completed and found to be in compliance with the approved plans and specifications. Final
acceptance of the project was given on December 18, 2012 after a final inspection was conducted
by HDR and County representatives. Gum Creek Farms, Inc. has submitted a final invoice along
with all required documentation and is requesting final payment including release of retainage.
BACKGROUND AND ANALYSIS: On August 29, 2011 Okaloosa County entered into a
contract with Gum Creek Farms, Inc. to perform the work associated with the Old Antioch Road
Improvements Project. Through the course of the project there were four change orders resulting
in a net increase of $59,669.79 with 15 added contract days. This total change order amount
includes balancing estimated plan quantities with final installed quantities used to complete the
project.
Gum Creek Farms, Inc. has requested that final payment be made for all work performed. A
final pay application for the contract balance of $54,106.95 is attached. This final pay request
will release all retainage to Gum Creek Farms, Inc. and cause the work to enter the one year
performance warranty period.
OPTIONS: Approve/Disapprove final payment to Gum Creek Farms, Inc. in the amount of
$54,106.95 for work associated with the Old Antioch Road Improvements Project and to
authorize the Chairman to approve the final change order (#4).
RECOMMENDATION: Approve final payment to Gum Creek Farms, Inc. in the amount of
