Contract Change Order No. 3 Project: Del Cerro II (MC-7) Public Supply Well Contractor: Layne Christensen Company Background: The development of the final well has proven that the water bearing formation continues to produce sand as was observed during the test well phase. Results indicate a gravel pack and screen is necessary to prevent sand and other deleterious material from migrating from the formation to the well. Gravel Packs and Screens are commonly used in unconsolidated aquifers and sometimes used in consolidated aquifers dependent on the water bearing formation’s lithology. Historically, Okaloosa County has not experienced wells that required gravel packs and screens and hence, the need for such was not anticipated and is deemed a changed condition from what was designed and bid. The gravel pack and screen is required for the Contractor to fulfill the performance standard set forth in the contract (Ref. paragraph 1.4 Section 02730L). In addition to adding the Gravel Pack and Screen, this change order revises the minimum capacity of the well from 1,000 gpm to 750 gpm. Furthermore, due to the problems encountered during the test well phase and now the need to add the gravel pack and screen, it is necessary to revise the contract time to allow for its procurement, installation, development, and testing including time to compensate for all work activities delayed for need of and dependent on the final well testing and final well hydraulic characteristics. Therefore, this Change Order adds 153 days to Substantial Completion and 122 days to Final Completion. Add the following line item to the contract: Item Description Est. Unit Unit Rate Total Cost Qnty 15 Furnish and Install Gravel Pack and Screen 1 LS $99 530.00 $99 530.00 as described in Contractor’s letter dated 4- April-2014 (copy attached) complete and in place for the lump sum amount of WATER +» MINERAL April 4, 2014 Mr. Roy Petrey, PE VP Florida Operations Polyengineering 102 Sunset Lane Shalimar, Fl 32579 RE: Well Screen Installation Quote - Okaloosa Del Cerro Dear Mr. Petrey, Layne Christensen Co. performed a preliminary pump test March 28, 2014 to clear the well and obtain sand concentration measurements. The preliminary test disclosed some good news and reinforced the fact the well is producing sand within the lime rock formation. The new production well yields 12.64gpm per ft. of drawdown @ 1125gpm and 13.63gpm per ft. of drawdown @ 750gpm. Compared to the test well results the yield increased by 125%. This increase helps reduce the up hole velocity and the stress on the formation while pumping water. The increased yield provided the ability to present a long term resolution removing the sand produced from the well. Layne Christensen Co. is recommending the installation of a Gravel Packed Stainless Steel Wire Wrapped Screen. Key Factors increasing value of well investment with Gravel Pack and Screen Installation: e Gravel pack and screen installation is required to meet the performance standard guarantee set forth in the contract (Ref. paragraph 1.4 Section 02730L) e Reduce the risk of erosion within the lime rock formation, eliminating the potential for an increase in sand production and undermining the cased area. e Helps eliminate the fine silts from entering the well and prevents turbidity issues while pumping. e Prevents premature wear in the pump bowl assembly e Based on current results the gravel pack and screen will eliminate the potential need for an above ground and below ground separator. WATER RESOURCES 4601 N. 252" Street, Valley, NE 68064 | Office: (402) 359-2042 | Fax: (402) 359-2310 | layne.com > ENERGY Original well design outlined a well capacity of 1000gpm. Installation of the gravel pack and screen will revise the minimum flow rate to 750gpm. The gravel pack and screen will not remove any form of minerals from the water nor will it eliminate the potential for water treatment. Layne Christensen is pleased to provide the following quote to furnish and install a Gravel Packed Stainless Steel Wire Wrapped Screen. Quote includes the following: e 16” x 197 ft. of Stainless Steel Wire Wrapped Screen (Slot size will be determined upon completion of sieve analysis) 16” x 40ft. of Carbon Steel lap pipe 16 estimated 3000Ib sacks of 20/40 gravel pack Installation of Screen and Gravel Pack Swab screen area with tight fitting swab and air lift to remove fines and settle gravel pack. Allowing gravel pack to properly settle into the screen area. e Installation and of Test Pump to develop well and test pump e Installation of (2) 10” Tee’s with Blind Flanges to distribution line Total Estimated Cost for above iteMs......... cc ccccceeeeeeeesessetees $99,530.00 Approval of the screen, ordering, delivery of the screen, installation, development and test pumping will create the need for additional contract time. The improved yield from the well may bring a change to other contract items adding to lead times for equipment. Layne Christensen respectfully request an extension of Substantial and Final Completion Time. Current Substantial Completion Date is June 10,2014 and Final Completion August 11, 2014 We would like to extend the Substantial Completion Date to November 10, 2014 and Final Completion Date to December 11, 2014 Chuck Rabold LAYNE | Water + Mineral + Energy Charles.Rabold@layne.com | layne.com yr J CY
