SUBJECT: 12th Avenue Sidewalks - LAP Supplemental

STATEMENT OF ISSUE: FDOT has issued Supplemental Agreement No. 1 to the 12th
Avenue Sidewalk Improvements LAP Project which increases funding by $32,930 for a total of
$203,748 including construction & CEI services.
BACKGROUND AND ANALYSIS: On March 19, 2013 Okaloosa County accepted a
$170,818 Local Agency Program (LAP) Agreement from the Florida Department of
Transportation (FDOT) to construct the 12th Avenue Sidewalk Improvements LAP Project. The
original LAP Agreement allocated funds for construction and CEI services.
Bids for construction of the project were received and opened on August 15, 2013. The lowest
bidder, Main Street Construction Services, LLC submitted a (corrected) bid of $186,665.50.
Total funding allocated for construction in the original LAP Agreement is $153,736.00. FDOT
has issued the attached Supplemental Agreement increasing construction funding to match the
low bid. It is requested that the Board of County Commissioners accept and execute four copies
of Supplemental Agreement No. 1 as well as the associated Resolution so that we can move
forward with the project.
OPTIONS: Approve/Disapprove execution of LAP Supplemental Agreement No. 1 for the 12th
Avenue Sidewalk Improvements Project.
RECOMMENDATION: Staff requests Board approval and execution by the Chairman of LAP
Supplemental Agreement No. 1 for the 12th Avenue Sidewalk Improvements Project.
