Request public comment and consideration concerning allowing Water & Sewer staff to self perform the construction of the lift station and wastewater force main that will serve Duke Field on Eglin Air Force Base. Undersheriff Don Adams, Okaloosa Sheriff’s Department & Jerry Sansom, Okaloosa School District, to address the Board concerning a proposed ordinance to fund the Teen Driver Challenge. 2. Resolution amending the Fiscal Year 2012 Budget, General Fund, to reflect appropriations from Florida Department of Transportation for Section 5311 rural public transportation grant in the amount of $157,350. 3. Request approval of a Public Event Permit for the Trek Women Breast Cancer Awareness Bicycle Ride, to be held on October 8, 2011, from 9:00 a.m. – 1:00 p.m., along various roadways in and around the Mary Esther and Fort Walton Beach areas, sponsored by Bob’s Bicycles. 4. Request approval of a Public Event Permit for a 5K Run, to be held on November 11, 2011, from 8:00 a.m. – 1:00 p.m., along a route in the Wright area of Fort Walton Beach, sponsored by Calvary Christian Academy & Preschool. 5. Request approval of final payment in the amount of $208,264.87 to CCI Systems, Inc. for completion of the 7th Special Forces Group SCADA fiber optic link project. 6. Request approval of a Resolution submitted by Surety Land Title of Florida, LLC for Okaloosa Island Leasehold Conversions to prepare Quitclaim Deeds for the following: Ronald L. Request approval of and authorize the Chairman to execute Change Order No. 1 with American Suncraft Construction for repair and painting of several area water tanks. District: All. b. Request approval of and authorize the Chairman to execute Task Order No. 54 with Constantine Engineering for the FY 2012 subsurface utility engineering services project. District: All. 5. John Hofstad/Public Works a. Informational update concerning the site development for the new Agriculture Extension Services building in Crestview. District: 1. b. Request approval of the selection committee’s recommendation for the Design & Construction Engineering & Inspection Services contract for the Sylvania Heights Resurfacing project. District: 4. c. Request consideration and staff direction concerning a request submitted by the Bluewater Bay (BWB) MSBU for assistance with construction of 4 sidewalks in BWB. District: 5. d. Request consideration and staff direction concerning proposed revisions to the department’s tree trimming policy. District: All. e. Request approval to schedule a public hearing on November 1, 2011 for public input concerning the adoption of an Illicit Discharge Ordinance related to non-stormwater discharges. District: All. f.
