BOARD OF COUNTY COMMISSIONERS AGENDA REQUEST DATE: November 15, 2016 TO: Honorable Chairman and Members of the Board FROM: Laura Porter SUBJECT: Acceptance and Utilization of Wellness Contribution Funds DEPARTMENT: Risk Management BCC DISTRICT: ALL STATEMENT OF ISSUE: Request the Board approve the Wellness Contribution Plan and authorize the Chairman to sign the related plan documents. BACKGROUND: On October 1, 2016 the County entered into an agreement for group health insurance with Blue Cross Blue Shield of Florida. The agreement includes a provision for annual wellness contributions to be made to the County for wellness initiatives driven by the Board of County Commissioners, Property Appraiser and Clerk of Court. The Wellness Contribution Plan specifies how the Wellness Contribution funds will be utilized. Employees are eligible for cash rewards for engaging in specified activities and wellness programs as a means to improve employees overall health and wellbeing. OPTIONS: Approve/Disapprove RECOMMENDATIONS: Staff recommends approval of the Wellness Contribution Plan and authorization for the Chairman to sign the related documents.
