yas halt re lacement included as Additive Alternate No. 1 and a crushed stone access roac att Tiza Jacks`o k as Additive ternate o. 2. On January 6, 2009, responsive bid packages were received from the following three contractors: Sikes Construction, NorthWest Florida Underground, LLC., and Starfish, Inc. The lowest responsive, responsible bidder was Starfish, Inc. with a total bid, including the two additive alternates, of $ 437,959. 67. The two Florida Forever grants total $456,435, leaving a remaining fund balance of $18,475 to cover the expense of possible change orders. Because the lowest responsive, responsible bidder' s price is under the available funding from the Florida Forever grants, Staff recommends awarding the base bid plus the two additive alternates to Starfish, Inc. in the not to exceed amount of $437,959.67. Recommendation: Staff requests City Council authorization to award ITB No. 09 -107 for the installation of five ( 5) stormwater pollutant separators to the lowest responsive, responsible bidder, Starfish, Inc. of Alabama, in the amount of $437,959.67, which includes the Base Bid ($ 424,849. 67), Additive Alternate No. 1 ($ 5, 835. 00), and Additive Alternate No. 2 ($ 7, 275.00). Staff further requests City Council authorization for the City Manager, or the City Manager' s designee, to execute individual change orders in the not to exceed amount of $18,475, which is the remainder of the balance of the Florida Forever grant award. If approved, sufficient funding is available in account 405- 8508- 334.36- 00 ( Florida Forever Grant).
