BOARD OF COUNTY COMMISSIONERS AGENDA REQUEST DATE: October 18, 2016 TO: Honorable Chairman and Members of the Board FROM: Jason Autrey SUBJECT: Oak Hill Road Repairs Change Order #3 DEPARTMENT: Public Works BCC DISTRICT: 1 STATEMENT OF ISSUE: Public Works is requesting Board approval of Change Order #3 for the Oak Hill Road Repairs Project, Contract C16-2403-PW, with Bullard Excavating, Inc. This change order will adjust several quantities to reflect a scope change by the design engineer after field observations during a rain event. BACKGROUND & ANALYSIS: As a result of field observation during, and immediately after, a rain event, the design engineer requested a change to the plans that would yield a better project for the County. The plan changes included removal of concrete swales, curb and gutter, pipe and concrete aprons and the addition of asphalt swales to the edge of roadway and rip rap at the bottom of the hill. In addition, this change order includes additional embankment to bring the site up to the design elevation due to continued erosion during the time between the initial site survey and construction. Change Order #3 is detailed in the attached schedule of quantities with a net increase of $23,468.00. Change Order #3 will bring the total percentage of all change orders to 13.5% and a total cost, including all change orders, to $499,647.50.
