Request public comment and consideration concerning a request submitted by Mark Way as agent for Continental Land, LLC for a change to the FLUM designation for a parcel of land from LDR to MU. If the FLUM amendment is approved, request for rezoning from R-1 to MU. The property contains 1.86 acres more or less and is located on the west side of Eglin Street in Fort Walton Beach. Request public comment and consideration of a petition submitted by Donald and Helen Jane Schroeder related to a Development Order Hearing for Beach Helicopter, a proposed helicopter sightseeing business requesting to be located in the Kelly Plantation area of unincorporated Okaloosa County. Request public comment and consideration concerning approval for the Water & Sewer Department to self perform construction of the Fort Walton Beach 1.7 mile 36” sewer force main project. Cory Wilkinson, AICP/CEP, HDR Engineering, to update the Board concerning the P.J. Adams Parkway Environmental Assessment and related public hearing scheduled for August 23 in Crestview. V. Request approval of a Public Event Permit for a 5k Fun Run/Walk, to be held on September 22, 2012, starting at Helen Back Café on Okaloosa Island, continuing west Rev. 08/16/12 Page 1 of 4  on Santa Rosa Island and returning to Helen Back Café, from 7:30 a.m. – 10:00 a.m., sponsored by the Okaloosa Law Enforcement Benevolent Association. 6. Request approval of a Public Event Permit for the EOD Memorial Challenge (5k march, 300m swim, 10k run), to be held on October 28, 2012, along Range Road from the SR 293 bypass to the back entrance of Rocky Bayou State Park, sponsored by the Chief Petty Officers Association for the Naval School Explosive Ordnance Disposal. 7. Request approval of a Special Event Permit to utilize a county beach area at the Boardwalk on Okaloosa Island for a cheerleading event, to be held March 23rd and 24th from 8:00 a.m. – 8:00 p.m., sponsored by Worldwide Spirit Association. 8. Request approval of a Proclamation designating September 17 – 23, 2012 as Constitution Week in Okaloosa County. 9. Resolution amending the Fiscal Year 2012 Budget, Airport Enterprise Fund, which reflected too much appropriations from Federal Aviation Administration for rehabilitation of the primary runway at Destin Airport in the amount of ($2,538,958). 10. Request approval of the assignment of a grant issued to the County for rehabilitation of the north apron at Bob Sikes Airport from the Department of Economic Opportunity to the Florida Department of Transportation. 11. Request approval of and authorize the Chairman to execute and submit an application for re-authorization of the current Large Area Artificial Reef Site permit which is about to expire. 12. Request approval of a request submitted by Honeycomb Holdings, LLC for acceptance of approximately 195’ of roadway improvements on Ida Coon Circle into the County’s permanent maintenance system and release of the related letter of credit. 13. Request approval to schedule a public hearing for comments related to the Water & Sewer Department self performing construction of the Camp Rudder pump station and force main project. 14. Request approval of and authorize the Chairman to execute Modification #7 to a grant agreement with the Department of Economic Opportunity for an extension of time on the Neighborhood Stabilization program. 15. Request approval for Purchasing staff to solicit quotes for purchase of roof coating materials and supplies necessary for repairs to multiple county buildings that have leaked during recent rains. 16. Request approval of and authorize the Chairman to execute a grant application for submittal to the Federal Aviation Administration for funding assistance with rehabilitating the Destin Airport runway. 17. Request approval of Task Order #2 with Zimmerman Agency for advertising services for Tourist Development. 18. Request approval to award the bid for food and beverage services at the Emerald Coast Convention Center to Aramark, they being the lowest bidder meeting specifications. Bid Amount – 17% of gross revenue of food & beverage sales. 19. Request approval to install signage for the “First Purple Heart County” designation along selected County roads in lieu of state maintained roads. 20. Request approval of and authorize the Chairman to execute Revision 1 to Engineering Task Order 53 for the 2012 Sunshine State One-Call of Florida subsurface utility engineering services project requirements. 22. Request approval of and authorize the Chairman to execute Task Order 59, Revision 1 with Gator Boring for directional bores related to the Rocky Bayou Force Main project. VI. Department Business 1. Kay Godwin/Human Resources a. Request consideration of the hiring committee’s recommendation for the position of Tourist Development Director. District: All. 2. Paul Lawson/Corrections a. Request consideration and staff direction concerning authorization to initiate the hiring process for five new correctional officers to begin work in FY13 due to an increase in the jail population. District: All. 3. Mike Stenson/Airports a. Request approval of and authorize the Chairman to execute Task Order #1 with Romjue Advertising for advertising support services through September 2012; also authorize a Request for Qualifications and request approval of a proposed recommendation committee to review proposals for an advertising contract. District: All. 4. Greg Donovan/Tourist Development a. Update concerning progress on improvements to departmental operations and ongoing revisions to policies and procedures. District: All. 5. Elliot Kampert/Growth Management a. Request consideration and approval of plans for Emerald Breeze Resort, a proposed hotel property including a 152 room hotel for Phase I, a 124 room hotel for Phase II and a 6,600 SF restaurant for Phase III, as submitted by Choctaw Engineering on behalf of Emerald Breeze Resort Group, LLC. The property is zoned Eglin and the FLUM designation is Eglin. The property is located on the south side of Hwy 98, east of Waterscape Condominium on Okaloosa Island. District: 2. b. Request consideration and staff direction concerning the Joint Land Use Study – Small Area Study. District: All. 6. John Hofstad/Public Works a. Request consideration and approval of a proposed conceptual Phase I site development plan for the remaining undeveloped 17 acres of county property on Okaloosa Island. District: All. b. Request approval of and authorize the Chairman to execute an Interlocal Agreement with the City of Fort Walton Beach for sharing of responsibilities and costs on the construction of a 1.7 mile 36” sewer force main. Request approval of and authorize the Chairman to execute a contract with Eglin Air Force Base for planning and construction of two pump stations and force mains to connect Eglin Main Base to the Arbennie Pritchett facility. District: 1,2&3. c. Request approval of and authorize the Chairman to execute Revision 1 to Engineering Task Order 59 for engineering design and construction services for the Eglin Main pump station and force main project. District: 1,2&3. d. Request approval of and authorize the Chairman to execute a contract with Eglin Air Force Base for planning and construction of a pump station and force main to connect Camp Rudder to the Arbennie Pritchett facility. Request Board authorization to accept Medicaid County billing retrospective final certification pursuant to section 409.915 F.S. for the time period of November 1, 2001 through April 30, 2012.
