AMENDMENT FIVE TO CONTRACT Contract No. C97-0066-WS Horizons of Okaloosa County Commercial Lawn Care — Water & Sewer This AMENDMENT THREE made and entered into this day of 2016, amends Contract No. C97-0996-AP, by and between Okaloosa County, Florida, a polmcal subdivision of the State of Florida (The County) and Horizons of Okaloosa County (the “Contractor”) for lawn care at the Destin-Fort Walton Beach Airport as follows: 1. The Contract is set to expire on January 31, 2016. 2. The parties agree to extend the Contract for three (3) months until April 30, 2016. 3. All other provisions of the initial Contract and subsequent amendments shall remain in full force and effect. C03-0996-AP exiension is approved for legal purposes. The draft extension for C97-0066-WS needs to be revised to reflect the correct Contract Number/identification code in the heading and in the body of the text. Currently it reads “C03-0066-WS” not “C97..". Once those revisions are made, that extension will also be approved for legal purposes. Let me know if you have any questions. Have a good day!
