AMENDMENT to CONTRACT
Horizons of Okaloosa County
C97-0066-WS, Landscape Maintenance
This Amendment is made by Okaloosa County Water & Sewer and Horizons of Okaloosa
County, parties to the agreement C97-0066-WS.
The Contract is amended as follows: The Okaloosa County Water & Sewer Department and
Horizons of Okaloosa County currently maintain the referenced contract for landscape
Maintenance. Horizons of Okaloosa County is proposing to maintain the current pricing for the
services provided at $45,624.00.
The Okaloosa County Water & Sewer Department has requested an addition to the Scope of
Services to include two (2) additional sites. (See Attachment 1, updated scope of service and
pricing).
1. Eglin Main Pump Station, Eglin Air Force Base.
2. 7 Special Forces Group Pump Station.
The change in scope of service will be an increase of $150.00/month for a total annual increase
of $1,800.00 for a total annual contract amount of $47,424.00.
The annual contract amount will be billed in monthly increments of $3,952.00.
This contract shail be renewable upon successful performance and written agreement by both
parties for additional one (1) year periods.
