STATEMENT OF ISSUE: Request approval of the final payment to Symetra in the amount of
$25,454.80.
BACKGROUND: On October 1, 2012, Okaloosa County Board of County Commissioners
entered into Contract #C12-1976-RM with Symetra to provide life, accidental death and
dismemberment and long-term disability insurance to its employees through September 30,
2014. On August 5, 2014, the contract to provide these benefits to employees was awarded to
another company effective October 1, 2014.
OPTIONS: Approve or Disapprove
RECOMMENDATIONS: Approve the final payment to Symetra for life, accidental death and
dismemberment and long-term disability insurance in the amount of $25,454.80.
