Dear Ms. Guarino:

This letter is to make you aware that you have exceeded the number of absences
allowed by the Code of Ordinances of the City of Fort Walton Beach and your seat on
the Historic Sites and Structures Board is declared vacant. You have missed attending
four regular meetings ( 3- 15- 05, 6-21- 05, 8- 16- 05, & 11- 15- 05) within a 12- month period
beginning January 2005. Please see the attached Historic Sites & Structures Board
meeting attendance chart. As the City Clerk, it is my duty to report the lack of
attendance to City Council.
Section 2. 02. 05 Vacancies of the City Code states:
Vacancies on any agency, authority, board, commission or committee shall occur
when a member fails to attend three successive regular meetings or a total of four .
regular meetinas or five special meetings within any 12- month period.
By copy of this certified letter it shall constitute evidence that a vacancy has occurred on
the Historic Sites & Structures Board and shall be authorization for the Council to
appoint another member for your unexpired term.
Enclosed is an application should you wish to be considered for reappointment. Please
complete and forward to City Hall for consideration.
The Mayor and Council appreciate your past service to the City and the Historic Sites
and Structures Board.
