WITNESSETH: That for and in consideration of payments, hereinafter mentioned, to be made by the Party of the First Part, the Party of the Second Part agrees to furnish Dental Insurance, Dental Option #3 in strict conformity with the provisions of this Contract, the Contractor’s response to RFP #44-14 (the “Proposal”), attached hereto as Exhibit A, as amended, and agrees to enter into a policy consistent with the terms as described in Exhibits A and B (negotiated Dental Option #3). The said Proposal, and the negotiated Dental Option #3, including the Policy that will be issued to the Owner is hereby made a part of this agreement as fully and to the same effect as if the same had been set forth at length in the body of this agreement. The parties agree that all matters of insurance are governed by the terms and conditions of the Policy and in the event of any conflict with this contract, the Policy shall control. in consideration of the foregoing promises, the Party of the First Part agrees to pay to the Party of the Second Part such unit prices for the insurance as are set out in the proposal in the manner provided in the Proposal beginning October 1, 2014. This contract shall be for a one (1) year period beginning October 1, 2014 and running through September 30, 2015. This contract may be renewed for one (1) additional one (1) year period with a maximum of a five (5) percent rate increase, and may be renewed for three (3) additional one (1) year periods at a rate agreed upon by both parties. Renewal notice is to be provided to the County of Okaloosa, Florida at least ninety (90) calendar days prior to the Anniversary Date.
