The Provider will be paid $1,000.00 per month for up to twelve (12) months, for a total contract amount not to exceed $12,000.00. Funds will be paid from Internal, General Band. The term of the agreement is July 15, 2014 through June 30, 2015. A copy of the agreement is attached. Public Content Request approval of Service Agreement #15-09 between Jamie Fields and The School Board of Okaloosa County, on behalf of Fort Walton Beach High School, to provide Color Guard Instruction. Instruction to include the following: 1) Proper performance techniques for dance, body and with various equipment; 2) assist Band Directors with costuming choices and creative planning; 3) responsible for rehearsals and performances of the students; 4) preparing routines and choreography The Provider will be paid $1,000.00 per month for up to twelve (12) months, for a total contract amount not to exceed $12,000.00. Instruction to include but not limited to dance, flag, rifle, sabre, swing flag and various props. Choreography to be used at football games, marching  contests, parades, and exhibitions. The Provider will be paid $6,000.00 for choreography instruction($3,000 to be paid at the end of August, and the remaining balance of $3,000.00 to be paid upon completion). The Provider will be paid $35.00 per hour for up to 110 hours ($3,850.00) for rehearsals and performances, for a total contract amount not to exceed $9,850.00.
