AGENDA REQUEST
DATE: April 5, 2016
TO: Honorable Chairman and Members of the Board
FROM: Jeff Littrell
SUBJECT: Polyengineering Task Order 6 Amendment 1
DEPARTMENT: Water and Sewer
BCC DISTRICT: All
STATEMENT OF ISSUE: Okaloosa County Water & Sewer (OCWS) has requested the
services of Polyengineering under Task Order 6 Amendment 1 per the terms of Contract #C13-
2067-WS. Amendment 1 (attached) adds $15,000.00 to the current Task Order amount, for a
new Task Order total of $55,000.00 for services in FY2016.
BACKGROUND: This amendment is requested to accommodate additional design phase
services required due to expansion of the scope of work covered under the OCWS Gravity Sewer
Rehab contract with Insituform. The referenced construction contract #C12-1955-WS has
increased from $288,383.00 (in 2012) to the current contract amount of $2,093,987.86 through
three change orders and a contract extension. The additional funds added to Task Order 6 for
Polyengineering will cover the remaining oversight of ongoing construction during the final
phase of the contract and all required contract closeout documents and pay requests.
Compensation will be for actual hours worked, plus direct expenses incurred.
OPTIONS: Approve or deny.
RECOMMENDATIONS: OCWS requests approval to authorize the Chairman to execute
Amendment 1 to Task Order 6, adding $15,000.00, for a total not to exceed fee of $55,000.00
