Before me, the undersigned Notary Public is and for the said County and State
personally appeared David deVaux, representing the Contractor, DeVaux
Construction Inc., who being duly sworn according to law deposes and says that all
labor, materials, and outstanding claims and indebtedness of whatever nature
arising out of the performance of the Contract with Okaloosa County, the Owner, for
Okaloosa County Wind Mitigation Project, (Installation of Storm Shutters/Window
Protection}Bid No. EM 26-12, (schools Baker #14, 15, 17, 18, Laurel Hill #8, &
Crestview High #11), have been paid in full and that upon receipt of the final
payment in the amount of $23,301.00, the Contractor releases and discharges the
Owner and his authorized representatives from any liens or claims of any nature
because of or arising from this Contract and/or its performance, which it has had,
has or may have in the future.
