AMENDMENT NO. 5
AGREEMENT
between
THE OKALOOSA COUNTY BOARD OF COUNTY COMMISSIONERS
and
OKALOOSA COMMUNITY DEVELOPMENT CORPORATION
for
UTILIZATION OF THE NEIGHBORHOOD STABILIZATION PROJECT (NSP1) FUNDS
This Amendment made and entered into this 2nd day of April 2013 by and between OKALOOSA COUNTY,
a political subdivision of the State of Florida having its principal office at 1804 Lewis Turner Blvd, Suite
206, Fort Walton Beach, FL 32547 (hereinafter referred to as "County") and OKALOOSA COMMUNITY
DEVELOPMENT CORPORATION, a non-for profit corporation having its principal office at 204 Cloverdale
Blvd, Fort Walton Beach, FL 32547 (hereinafter referred to as "Developer").
NOW, THEREFORE, in consideration of the mutual benefits contained herein, the County and Developer
agree as follows:
ATTACHMENT I – Activity Budget Summary and Work Plan
Deleted in its entirety and amended through Reconciliation of Activity Budget Summary and Work
Plan through Modifications to Grant Agreement Number 10DB-4X-01-56-01-F 18 between
Okaloosa County and The State of Florida Department of Economic Opportunity. The following
modifications are hereby incorporated;
Modification 7, executed October 29, 2012, Exhibit 1
Modification 8, executed March 8, 2013, Exhibit 2
Section 11. Term:
Is hereby revised to reflect an ending date of September 30, 2013.
All provisions not in conflict with this Amendment remain in full force and effect, and are to be
performed at the level specified in the agreement.
IN WITNESS WHEREOF, the parties hereto have caused this Agreement to be executed by their duly
authorized officials/ representatives on the day and date indicated below:
