Commercial Landscape Maintenance
This AMENDMENT TWO made and entered into this _ day of December 20135,
amends Contract No. C97-0066-WS, by and between Okaloosa County, Florida, a political
subdivision of the State of Florida (the “County”) and Horizons of Okaloosa County (the
“Contractor”) for landscape maintenance services for Okaloosa County Water & Sewer
Department as follows (the “Contract™):
1. The Contract is set to expire on December 31, 2015.
2. The parties agree to extend the Contract for one (1) month until January 31, 2016.
3. All other provisions of the initial Contract and subsequent amendments shall remain in
full force and effect.
IN WITNESS WHEREOF, the parties hereto have caused these presents to be executed
on the day and year first hereinabove written.
