Dunlap 90 College Blvd E Niceville, Florida 32578 RE: Contract C08-1674-PS Amendment 8 - Explanation of Changes and Potential Changes in the Monthly Invoice on Account 311664036 Dear Daniel, Good afternoon, and as requested, please find below the explanation for the changes in the monthly invoice as well as services and features that are subject to change in the future on Account 311664036. As discussed, the following items are to be amended for contract C08-1674-PS, and the amendment should have consideration beginning for the September 2013 billing and for any billing received thereafter: 1, 3.0M Month LAN 3MG — This is a service that is governed by the tariff, and the service/rate is non-contractual. The rate is subject to change on the monthly invoice based on a rate change with this service in the tariff after a periodic evaluation by CenturyLink of its products, prices and infrastructure. This feature has been noted on the 911 Network Services Contract in the past to indicate these features are being used by Okaloosa County 911 as well as to help reconcile the monthly invoices. 2. HSI Modem Wired - This is a service associated with the 3.0M Month LAN 3MG that is governed by the tariff, and the service/rate is non-contractual. The rate is subject to change on the monthly invoice based on a rate change with this service in the tariff after a periodic evaluation by CenturyLink of its products, prices and infrastructure. This feature has been noted on the 911 Network Services Contract in the past to indicate these features are being used by Okaloosa County 911 as well as to help reconcile the monthly invoices. Based on the evaluation, CenturyLink increased the price from $5.49 to $6.49 in the September billing cycle. 3. Static IP - This is a feature associated with the 3.0M Month LAN 3MG that is governed by the tariff, and the feature/rate is non-contractual. The rate is subject to change on the monthly invoice based on a rate change with this service in the tariff after a periodic evaluation by CenturyLink of its products, prices and infrastructure. This feature has been noted on the 911 Network Services Contract in the past to indicate these features are being used by Okaloosa County 911 as well as to help reconcile the monthly invoices. 4. 911 ANI/ALI per 1000 — Annually, CenturyLink audits its database (wireline records only) to take a snapshot of the changes that have occurred in the record count for each county in the State of Florida. Due to the nature of the telecommunication industry, wireline records are decreasing continuously while wireless (post-paid and pre-paid), VoIP and CLEC records are increasing. Therefore, CenturyLink makes this adjustment on the invoices to reflect this decrease in record count in our database that CenturyLink is responsible for managing and maintaining. Based on CenturyLink” the annual database audit, CenturyLink decreased the price from $4,650.00 to $4,092.00 and it should be reflected on the October 2013 billing and moving forward. 5. Centrex Lines - It is important to note that the Centrex Lines utilized by Okaloosa County 911 are subject to and governed by the Okaloosa County Centrex Contract. We have included the Centrex Lines on the 911 Network Services Contract in the past to indicate these services are being used by Okaloosa County 911 as well as to help reconcile the monthly invoices. 6. Additional Charges - Rates do not include applicable local, state, or federal taxes, fees, or surcharges that CenturyLink may bill Customer. 7. Enhanced Voice Solution — This is a usage based long distance service that will fluctuate monthly. 8. Remote Call Forwarding — This is a feature that is governed by the tariff, and the feature/rate is non-contractual. The rate is subject to change on the monthly invoice based on a rate change with this feature in the tariff after a periodic evaluation by CenturyLink of its products, prices and infrastructure. This feature has been noted on the 911 Network Services Contract in the past to indicate these features are being used by Okaloosa County 911 as well as to help reconcile the monthly invoices.
