Box 4009, Fort Walton Beach, FL 32549 Original Message---- - From: Estelle Jones Sent: Wednesday, February 17, 2010 4: 17 PM To: Samantha Abell Cc: Michael Beedie Subject: FW: TBA at 25 U.S. Highway 98 - Ft. Walton Beach Michael /Samantha, Attached is the proposed scope of work for 25MSP to be funded and managed by EPA Region 4 staff. Work is tentatively scheduled to begin the week of May 10. EPA has asked for comments on the Scope from City staff by this Friday. The summary is located on page 1 in the first paragraph and essentially includes a more extensive Phase II, complete removal of the UST's, required closure assessment report and other documentation required to be submitted to the state ( FDEP), soil and water samples and analysis, and a detailed clean -up options and cost estimates report. Typically UST removals are not included as part of these grants, but to our advantage, was added to the SOW after speaking on the phone more extensively about the project. I have reviewed the entire scope and only have 1 minor comment: that the project team be advised the site is located very close to the Sound and there is a fairly high water table. IPVI Other than that, I just need to e -mail her back by this Friday am to confirm we approve of the SOW. Walton Beach Hi Stella and Alex, Please read over the Statement of Work I have prepared for the upcoming TBA and let me know if you think I should add or delete any language. Please give me any comments by this Friday (2/ 19) so I can send it to EPA Headquarters on Monday. If you need more time please let me know. See attached file: TBASOW2009petrol- FWB. doc) Also, Alex can you let me know who at FDEP I should contact about getting access to any files on this site. Thanks, Margaret Margaret A. Olson Environmental Engineer Brownfields Program USEPA Region 4 404) 562 -8601 24- SOW) STATEMENT OF WORK FOR A COMBINED PHASE II ENVIRONMENTAL ASSESSMENT AND UNDERGROUND STORAGE TANK REMOVAL AND CLOSURE ASSESSMENT AT 25 U.S. Highway 98, Ft. Walton Beach, Florida The EPA Project Officer (TOPO) for this Phase II assessment and UST Removal and Closure Assessment is: Margaret Olson EPA Region 4 Brownfields 404 -562 -8601 olson.margaret@epa.gov Objective: Conduct a more extensive Phase II Environmental Site Assessment ( ESA) at the subj ect property in accordance with ASTM E -1903 -97 ( 2002) " Standard Practice for Environmental Site Assessments: Phase II Environmental Site Assessment Process ". In addition, conduct an Underground Storage Tank (UST) Removal and a Closure Assessment according to the Florida Department of Environmental Protection, Storage Tank System Closure Assessment Requirements in Chapter 67 -761, and Florida Administrative Code ( F.A.C.). After the tanks have been removed and sample results are obtained, a site -specific Analysis of Brownfields Cleanup Alternatives (ABCA) describing clean up options and cost estimates for the clean up will be developed. Background: The 25 U.S. Highway 98 ( the site) will require removal of four (4) 10, 000 - gallon underground storage tanks. A Phase I ESA was conducted in August 2006. A Phase II ESA was also conducted in August 2006 and concluded that there was soil contamination that could not be quantified without tank removal. Tank Removal Justification: In order to adequately assess the soil contamination and leakage from the underground tanks, it will be necessary to remove all tanks in order to accurately fill in data gaps and thoroughly assess this site. Experience: The contractor should demonstrate and discuss experience with UST removals and assessments in the State of Florida. Start Date: Administrative tasks ( QAPP, Health and Safety Plan, file review, etc.) should be initiated as soon as Task Order is issued. Field activities should be scheduled with the TOPO but are expected to begin by the week of May 10, 2010 unless otherwise negotiated with the TOPO. 25- Tasks Section l: 1. File Review - Review the limited site information as provided by the TOPO (Phase I and previous Phase II ESAs). A request has already been made for any additional information held by the Florida Department of Environmental Protection ( FDEP). All forthcoming information will be sent to the contractor as it is received. 2. Site Visit —It is anticipated that a preliminary visit ( 1) to the site may be necessary prior to preparation of the Quality Assurance Project Plan (and sampling plan). Contact TOPO and determine whether pre -sampling site visit will be necessary based on all available information about the site and site conditions. Subsequently, all field sampling activities will be conducted during the tank removal and sampling event on the date of which will be negotiated with the TOPO. 3. Site Access —Access will be obtained by TOPO. Tasks Section 2: 4. Quality Assurance Project Plan ( QAPP) - Prepare draft combined site -QAPP for TOPO review. QAPP will incorporate the tank removals /field sampling/work plan as one project. All QA/QC procedures shall be in accordance with applicable professional technical standards, EPA requirements, government regulations and guidelines, and specific project goals and requirements. 5. Health and Safety Plan (HASP) - Develop a health and safety plan for field activities at the combined site. The contractor shall implement and manage a Health and Safety program plan in compliance with all requirements of EPA and the Occupational Safety and Health Administration ( OSHA) 29 CFR 1910. 120 and 121 for activities at hazardous waste sites and other health and safety requirements as appropriate. Tasks Section 3: 6. Field Activities - Perform all necessary field activities including tank removals, disposal of soil incidental to tank removal, and sampling in accordance with the approved QAPP and HASP. If asbestos -containing materials (ACM) are present, an accredited asbestos inspector will be present at site during sampling operations. Field activities will be limited to a preliminary (scoping) visit and a tank removal /field sampling trip, unless otherwise negotiated with the TOPO. 7. Removal of Tanks - The tanks should be removed by a Florida Certified Pollutant Storage System contractor with a disposal certificate. 8. Contaminated Soil Disposal —Provide laboratory analysis, monitoring equipment, and documented soil disposal in accordance with the Florida Department of Environmental Protection, Storage Tank System Closure Assessment Requirements in Chapter 67 -761, and Florida Administrative Code ( F.A.C.). Identify the monitoring 2 26- equipment that will be used and provide documentation of excavation of contaminated soil and clean fill. A maximum of 200 cubic yards of contaminated soil is allowed per excavation site. 9. Analysis - Coordinate analytical support needed to analyze any samples taken at the site. Laboratory analysis will be procured through the contractor. Unless directed otherwise, all lab analysis should be procured with fourteen ( 14) day turnaround. Contractor will also perform necessary data management and data validation. A formal data validation package is not required however the contractor must submit an evaluation of the laboratory data which includes checking the chain of custody records for sampling, shipping, and analysis, reporting for accuracy and completeness, and a review of laboratory quality control data which includes a check of data validation worksheets for organic and iriorganic analysis, completeness assessment, sample preservation and holding times, instrument performance check, initial and continuing calibration blank analysis, organic surrogate spike recoveries, internal standard performance, field duplicate precision, ICP interference and ICP dilution for metal analysis. 10. Risk Analysis —Assess risks associated with sample data by comparing analytical data to FDEP Soil Cleanup Target Levels ( SCTLs) and Groundwater Cleanup Target Levels ( GCTLs) and other applicable state standards, Regional Screening Levels and Maximum Contaminant Levels ( MCLs). Please see " Regional Screening Levels for Chemical Contaminants at Superfund Sites" website at htlp:// gpa- prgs.oml.gov/chemicals/ index.shtml for updated information on determining screening levels during risk assessment). 11. Waste Management - Dispose of investigation derived wastes, per applicable EPA guidance documents ( Managing Investigation Derived Wastes for Site Inspections, OSWER Directive 9345. 3 -02) or FDEP guidance documents. In addition, dispose of excavated soil incidental to the tank removals. 12. Other Support - Perform other activities necessary to complete the Phase II assessment, including providing assistance in necessary scoping meetings with the EPA, TBA requestor and other stakeholders, including state officials when necessary. 13. Property Profile Form - Complete and submit relevant portions of the required EPA Property Profile Form reporting the commencement and /or completion of any Phase II activities. This form should be submitted in both hard copy and electronic copy. 14. Prepare Site Analysis of Brownfield Cleanup Alternatives (ABCA) - Complete a draft and final report summarizing cleanup options and cost estimates for cleanup of residual contamination of the sites. An ABCA will be applicable if residual petroleum contamination is found from leaking tanks. 3 27_ Tasks Section 4: 15. Deliverables to TOPO —All draft
