H.E.R.O. Feedback Form Humanitarian Efforts Recognized by Okaloosa County (To be completed by Department Director) Employee’s Full Name: Tim Busby Department: Public Safety/Emergency Medical Services Division Job Title: EMT-ALS Detail reasons why Director is nominating employee for a H.E.R.O. Award: On 02/03/15 Tim Busby and his partner were the assigned crew of OCEMS Medic 4. They were attached to a transfer from North Okaloosa Medical Center to the University of Alabama-Birmingham Medical Center. The crew picked up their patient and began the transport which was uneventful until they got to Fort Deposit, Alabama. At that point the Medic Unit sustained a mechanical breakdown causing the crew to pull off at the exit and seek assistance from the local ems agency in completing the transport. While awaiting for the transport to arrive, the crew witnessed a serious, near head on vehicle crash. Their were four occupants in the two cars that were involved. Three of the four sustained major injuries. Tim immediatley took action to assist the four victims until local resources could arrive. At the same time he was attentive to the patient in the patient compartment of Medic 4 and was able to successfully care for her as well as the four involved in the motor vehicle crash. By keeping a calm demeanor and thinking clearly, Tim was able to pass on an excellent report to the local incident command and request the proper resources (two medical helicopters and two ground units) needed to facilitate transport of the patients to trauma centers in the area. By internalizing and projecting the Department's mission wherever he is, and by providing exemplary patient care and high level scene management under very difficult circumstances, Mr. Busby acted in a heroic manner. Amid the chaotic scene, he ensured the patient from North Okaloosa was well cared for and delivered safetly to UAB and the other four patients were stabilized and delivered safely to the appropriate area hospitals and trauma centers.
